Nottingham IVC Full Membership Application Form

Please read the following instructions in full before completing the application form.

The information on this form is confidential and will not be divulged to anyone outside the committee.

The committee cannot consider your application unless submitted with a photograph. If you are unable to supply a photograph, please arrange to have one taken by any committee member at club night.

You will receive written notification if your application has been successful. A list of new members is also published in the bulletin each month.

Data Protection Act 1998: The data supplied by you is stored in accordance with the Data Protection Act 1998 and is kept secure from loss, unauthorised disclosure or access. Such data is only used for the purposes of administration of Nottingham IVC and will not be sold, transferred or shared other than in accordance of the constitution of the club. Your application indicates that you accept this.

Checklist:

1. Please type in the (brackets) on the form. 2. If sending application via post, please attach a photograph with your name clearly printed on the reverse. If sending via email, attach a photograph electronically. 3. Either: Email the form to the New Members Officer and hand your cheque to any committee member during club night); or: Print the form out, attach a cheque (NOT cash) made payable to ‘Nottingham IVC’ for the appropriate membership fee, and return the form to the New Members Officer (or any other committee member on club night). The committee cannot consider your application unless the cheque has been posted, or handed to a committee member. 4. Evidence should be included with your application if you are eligible to pay the concessionary rate. 5. Please try to make yourself known to at least two committee members so that your application can be given full and proper consideration.

Membership Fee:

Memberships are approved at the end of each month (excluding December). If your payment is received on or before the Tuesday Club Night which coincides with the bulletin deadline, for the month in which your temporary membership expires, then you will be entitled to a £5 discount against the full amount.
