LIFETIME FITNESS GREEN VALLEY MASTERS INVITE - 2017 OUTDOOR POOL GVLT, 121 Carnegie St., Henderson, NV FRIDAY, APRIL 7TH, 2017 HOSTED BY LIFETIME FITNESS, NON-SANCTIONED EVENT Warm-up 4:30pm, Meet Start 5:30pm.

Entries You may enter up to 4 individual events. Entries must be received by Friday, May 5 TH. Deck/Late entries may be turned in and will be considered on a case by case scenario. Late entries may be entered in events with NT regardless of submitted times. Lifetime Members vs. Non-Members: This meet is an invite meaning lifetime non-members will be welcome to participate. We will require names and ID’s for entry into the club on the day of the meet. Current members are advised to enter the club normally. The meet will be hosted on the outdoor pool deck. Instruction/Direction through the club will be provided upon entry. Our outdoor Bar/Bistro area will be open to all participants during and after the event.

Course: Outdoor 25 Meter with 6 Athletic lanes (Width is smaller than average competition lanes). Warm up in main pool, all 6 lanes. Pool depth: Start 5ft 5in; End 4Ft. WARM-UP Procedure: All lanes open for warm up under Official Supervision between 4:30pm – 5:15pm. Diving will be allowed in designated lanes during designated times only under Coach Supervision. Dive Lanes will be open during the last 15 minutes of warm up. Pool Closes 15 minutes prior to meet start at 5:30pm. MEET REFEREE: Will be in charge of the meet. Any questions regarding the conduct of the meet should be directed to the Referee. RULES: Meet will follow basic meet guidelines. Due to the meet being a non-sanctioned meet there will be no stroke/turn officials, only a starting official will be present. In the case that the facility cannot produce timers we will be relying on the meet participants to help out where they can. Times will be recorded but cannot be used as official times since this is a non- sanctioned meet. There will also not be any awards handed out for this meet because of the relaxed nature of it being a non- sanctioned meet. Events will be combined in an open event format and will be separated into respective age-groups following the end of the meet. RECORDING DEVICES & MEDIA NOTICE: The use of any device is strictly prohibited anywhere from behind the blocks or in the locker rooms for any reason. Photographs, videos, and media may be allowed under appropriate circumstances. Entry into the meet is acknowledgement and consent to the fact that there may be this time of media at the meet and all/any participants could be subject to this. DRONES: No drones allowed. SWIMWEAR: Swimwear must be appropriate. Clothes that bleed or come apart in the pools are prohibited along with anything not appropriate for a family facility. Full body suits will also be prohibited. Subject to ruling on a case by case situation at the facilities discretion. DECK CHANGES: …Prohibited. We have locker rooms. RACING STARTS: Blocks are available for racing starts. Any participant uncomfortable with diving or anyone unsafe with diving will be starting from a push in the water. ELIGIBILITY: Open to all athletes with or without a USMS or USA Swimming Registration. Athletes must be 18 years of age or older to participate. Entry form must be received by the meet entry deadline in order to participate. Late application will be considered and subject to monetary penalties. SUBMITTED TIMES: Times submitted will be used for event/heat seeding purposes. Times should be declared with Y = yards, M = Meters, or LC = 50 Meters. Any athlete without a time or who is unsure should enter events with NT = No Time. Seeding will done in a fastest to slowest style. AWARDS: No awards will be given or received. This is a non-sanctioned meet hosted by Lifetime for the benefit of the entered athletes. ENTRY FEES: $10.00 for each individual swimmer along with a $4.00 per event. Payment must be in the form of Check, Cash, or Credit and accompany each individual entry form. Payments in the form of Cash must be exact as there will not be any change for monies exceeding the total cost of entry. Checks should be made payable to Lifetime Fitness with the exact amount due. Credit Card information should be completely filled out on the entry form itself. Lifetime Members will have the option to pay via club tab. Failure to cover the full cost of the individual’s entry form entry fee, Returned checks, or inaccurate payment information may result in scratched events or the discard of said individuals entry form. Email verification will be sent to any individuals entering the event. There will be NO REFUNDS for submitted forms. DELETIONS/SCRATCHES will not be refunded.

It is understood and agreed that lifetime fitness shall be free from any liabilities or claims for damages arising by reason of injuries to anyone during the conduct of the event. Cancellation Policy:

There is inherent risk of injury, whether caused by me or someone else, in the use of or presence at a Life Time Fitness center, the use of Life Time Fitness’ equipment and services, and participation in Life Time Fitness’ programs. This risk includes, but is not limited to, (a) injuries arising from the use of any of the Life Time Fitness centers or equipment, including any accidental or “slip and fall” injuries; (b) injuries arising from participation in supervised or unsupervised activities and programs within a Life Time Fitness center, to the extent sponsored or endorsed by Life Time Fitness; (c) injuries or medical disorders resulting from exercise at a Life Time Fitness center including, but not limited to, heart attacks, strokes, heart stress, sprains, broken bones and torn muscles or ligaments; and (d) injuries resulting from the action taken or decisions made regarding medical or survival procedures.

I understand and voluntarily accept this risk, I agree to specifically assume all risk of injury, whether physical or mental, as well as all risk of loss, theft or damage of personal property while I am using or present at any Life Time Fitness center, using any lockers, equipment or services at any Life Time Fitness center or participating in Life Time Fitness’ programs, whether such programs take place inside or outside of a Life Time Fitness center. I waive any and all claims or actions that may arise against LTF Club Operations Company, Inc. (“LTF Operations”), its parent companies, affiliates, subsidiaries, successors, assigns, independent contractors and agents as well as each party’s owners, directors, employees or volunteers as a result of any such injury, loss, theft or damage, including and without limitation, personal, bodily or mental injury, economic loss or any damage resulting from the negligence of LTF Operations, its parent companies, affiliates, subsidiaries, successors, assigns, agents, independent contractors or anyone else using a Life Time Fitness center. If there is any claim by anyone based on any injury, loss, theft or damage that involves me, I agree to defend LTF Operations, its parent companies, affiliates, subsidiaries, successors, assigns, agents and independent contractors against such claims and pay such parties for all expenses relating to the claim, and indemnify LTF Operations, its parent companies, affiliates, subsidiaries, successors, assigns, agents and independent contractors for all obligations resulting from such claims. I hereby irrevocably consent to and grant Life Time Fitness, Inc., its subsidiaries, agents and representatives (“Life Time Fitness”), the exclusive and unlimited right to use and reproduce any and all photographs, audio recordings, video recordings or testimonial accounts taken by Life Time Fitness that contain my person, name, image, voice, likeness or account, for any lawful purpose whatsoever and using any means available, including but not limited to, any Life Time Fitness corporate or marketing communication or materials. I waive the right to inspect, approve or edit any such use or reproduction, and Life Time Fitness may make any and all changes, modifications, rearrangements, additions or deletions in its use or reproductions without any approval. Policies (subject to change) 1. Class fees are nonrefundable, except in the following circumstances: a. For medical disabilities, a prorated refund or credit will be given when participant provides a doctor’s note stating they cannot participate in the program. b. If a class is canceled, every effort will be made to find a suitable class for the participant who has registered for the canceled class. If a suitable class cannot be found, the participant will be provided a full refund. If a refund is granted, the refund will be issued in the form of a credit on the membership account. 2. We will follow the Child Center’s policy to exclude sick children. 3. Participant’s Authorized Adult must remain on Life Time’s premises and supervise the Participant in the locker room before and after swimming classes for any wardrobe-changing activities related to swimming. 4. I agree and understand that meet fees for meets that I sign my child up for will be charged on my club tab on the date the entries are due.
