Due to your counselor ASAP. This form will help us accurately tell an admissions officer the type of person you are. We are looking for ways to describe you without parroting back the usual information most recommendations include like GPA, activities and honors. Please carefully consider your responses to the following questions as they are extremely valuable in helping us assist you through your college admissions process. The more information you provide, the stronger the letter of recommendation. In order to strengthen your recommendation, please TYPE the answers to the following questions and attach the answers:
Resume Please attach a resume that lists activities, community service, employment and any honors you may have received. Include any leadership positions you may have held, currently hold, projects you have been involved with in any activity. Finally, list the grade and number of hours a week you participated in the activity. Please also include any summer programs you have attended.

Colleges want to get to know you outside of your stats (GPA, test scores) and see what makes you unique so tell us something important or whimsical about you that this questionnaire may have missed, or leave us with an anecdote that you feel reveals something about your personality. Please be open and candid, and feel free to be creative. It need not be structured; stream of consciousness writing is fine.
