About this Unit For this Unit you need to be able to prepare for and undertake extended feeding techniques other than oral feeding. These methods include, Percutaneous Endoscopic Gastrostomy (PEG) feeds, naso-gastric (NG) tube feeds and sub-cutaneous fluid infusions. This unit does not cover intravenous techniques for giving fluids.
Scope The scope is here to give you guidance on possible areas to be covered in this Unit. The terms in this section give you a list of options linked with items in the performance criteria. You need to provide evidence for any option related to your work area.
Adverse reactions can include: pain; regurgitation; vomiting.
Feeding procedures can include: Percutaneous Endoscopic Gastrostomy (PEG) feeds; naso- gastric (NG) tube feeds; sub-cutaneous fluid infusions.
Electrical equipment can include: pumps; infusion monitors.
Environment includes: maintaining privacy and dignity; reducing noise and ensuring adequate heat; space; light.
Equipment may include: syringes; spigots; jugs; litmus paper.
Key people include: family; friends; carers; others with whom the individual has a supportive relationship.
Standard precautions and health and safety measures including handwashing/cleansing before, during and after the activity; the use of personal protective clothing and additional protective equipment; handling contaminated items; disposing of waste; safe moving and handling techniques and untoward incident procedures.
Your knowledge and understanding for this Unit will relate to legal requirements and codes of practice applicable to the scope of your work and others with whom you work; the nature of the work you are undertaking; your role and level of responsibility within your organisation (eg whether you have responsibility to support the work of others); the individuals, key people1 and others with whom you are required to work and the degree of autonomy you have for the management of your own work activities.

Values underpinning the whole of the Unit The values underpinning this Unit have been derived from the key purpose statement2, the statement of expectations from carers and people receiving services, relevant service standards and codes of practice for health and social care in the four UK countries. If you are working with children and young people they can be found in the principles of Care Unit HSC34. If you are working with adults they can be found in HSC35. To achieve this Unit you must demonstrate that you have applied the principles of care outlined in either Unit HSC34 or HSC35 in your practice and through your knowledge

SPECIFIC EVIDENCE REQUIREMENTS FOR THIS UNIT
Simulation: Simulation is NOT permitted for any part of this unit. The following forms of evidence ARE mandatory: Direct observation: Your assessor/expert witness must observe you in real work activities which provide evidence for a significant number of the performance criteria for each element of this unit. Ideally you should be observed while carrying out a procedure from the preparation stage, through to feeding an individual and then any records which may have to be completed. Reflective accounts/professional discussion: These will be a description of your practice in particular situations of how you take standard precautions when carrying out extended feeding techniques. Or, you may wish to describe an incident when you became aware of adverse reactions and what you did about it.
Competence of performance and knowledge could also be demonstrated using a variety of evidence from the following: Questioning/professional discussion: may be used to provide evidence of knowledge, legislation, policies and procedures which cannot be fully evidenced through direct observation or reflective accounts. In addition the assessor/expert witness may also ask questions to clarify aspects of your practice. It is unlikely that this Unit could be completed without additional questioning, as the assessor/expert witness may want to check that you know a larger variety of techniques than those you are using with individuals with whom you are currently working. Expert Witness: A designated expert witness (maybe a nurse or dietician) may provide direct observation of practice, questioning, professional discussion and feedback on reflective accounts to provide evidence of specialist skills. Witness testimony: can be a confirmation or authentication of the activities described in your evidence which your assessor has not seen. This could be provided by a work colleague or service user. Products – Suggestions:- Organisational policy and procedures in relation to Health and Safety aspects of this Unit, such as protective clothing, handling of contaminated products etc and case records containing the feeding needs of individuals. These do not need to be included in your portfolio however they will form the basis of discussion with your assessor and need to be available for the IV to authenticate. Any products which are included must be anonymised.
GENERAL GUIDANCE
Prior to commencing this unit you should agree and complete an assessment plan with your assessor which details the assessment methods you will be using, and the tasks you will be undertaking to demonstrate your competence. Evidence must be provided for ALL of the performance criteria ALL of the knowledge and the parts of the scope that are relevant to your job role. The evidence must reflect the policies and procedures of your workplace and be linked to current legislation, values and the principles of best practice within Health and Care Settings. This will include the National Service Standards and/or KSF for your areas of work and the individuals you care for. All evidence must relate to your own work practice.

Care and support 10 A working understanding of the impact this type of feeding will have on individuals. 11 A working understanding of a variety of conditions and situations where feeding may be undertaken by extended methods. 12 A working understanding of other staff who may be involved with the nutritional and fluid intake of individuals.

Materials and equipment 17 A working understanding of the equipment and materials required for the types of extended feeding techniques relevant to your work area. 18 A working understanding of the different types of feed available for the types of extended feeding techniques relevant to your work area. Procedures and techniques 19 A working understanding of how a variety of feeding tubes are put in place and the preparation of the individual for the procedure. 20 An in-depth understanding of how to monitor, improve and maintain nutritional and fluid needs of individuals. 21 A working understanding of methods used to ensure the extended feeding equipment is correctly positioned prior to feeding. 22 A working understanding of the importance of maintaining the correct level of cleanliness for extended feeding techniques. 23 A working understanding of the importance of following procedures for extended feeding techniques exactly as specified, and the potential effects of not doing so. 24 A working understanding of the importance of packing up used equipment and materials and covering receptacles containing body fluids prior to leaving the immediate care area. 25 A working understanding of how and where to dispose of: (a) used equipment, materials and feeds (b) body fluids aspirated prior to feeding Records and documentation 26 A working knowledge of the importance of immediately reporting any issues which are outside your own sphere of competence without delay to the relevant member of staff. 27 A working understanding of: (a) the importance of keeping accurate and up to date records (b) the specific records required for reporting on gastric aspirate and the removal of nasogastric tubes

HSC357.2 Carry out and report on extended feeding procedures Performance criteria DO RA EW Q P WT 1 You apply standard precautions for infection control, any other relevant health and safety measures. 2 You involve the individual and key people throughout the procedure. 3 You ensure the feeding tube is in the correct position according to the type of tube being used. 4 You carry out the extended feeding: (a) at an appropriate time according to the individual’s plan of care (b) using appropriate techniques (c) using equipment in line with manufacturer’s instructions (d) in a manner which optimises the patient’s comfort and dignity and minimises pain and trauma 5 You observe the individual throughout the activity, recognise and report any condition or behaviour which may signify adverse reactions to the activity and take the appropriate action. 6 You ensure that adequate and relevant fluids, feeds and equipment are stocked in the care environment to meet immediate future needs, re-ordering as appropriate. 7 You ensure the extended feeding tubes are securely attached in a way that prevent discomfort and promotes dignity of the individual.
