Transportation: Hastings School Bus (Cost to ride the bus is $3.00 presale or $4.00 if purchase at door) Please keep above information.

Please complete and return the following form to a ski club sponsor if you would like your student to be able to ride the school bus on any of the above dates. No student will be able to ride unless he/she returns this form.

______has my permission to take part in this extra-curricular trip to Bittersweet. I understand that school rules continue to apply to students while they are participating in an extra-curricular trip, and I assume full responsibility for the actions of my son/daughter. I realize that if my student’s behavior is not consistent with school rules and he/she is not willing to cooperate I will be called to pick him/her up at Bittersweet. Furthermore, I agree that the Hastings Area School System and its employees or sponsors of this trip will not be held responsible for accidents, injuries, and or other occurrences beyond their control, and in the event that my son/daughter requires medical treatment while on the trip, I authorize the trip sponsor to act on my behalf in securing medical attention deemed necessary.

I understand that every reasonable effort will be made to plan for the safety of this trip, and it is my understanding that the students will be transported by school bus.
