Accreditation Survey Discussion 1125 - Community Health Centers- Ariston Julian - Any others? Thank you to Ariston and Evan from Community Health Centers for providing the group with some information on their recent Joint Commission survey. Hazard Vulnerability Assessment (HVA) 1135 - Kaiser 2017 model- updated/instructions - Discussion- who has used it? Any recommendations? HVA templates can be found on the Resources webpage and supporting resources to help your facility conduct your HVA: http://cosb.countyofsb.org/phd/disasterprep.aspx?id=53148 Remember to look at the surrounding community and area around your facility you can use the Santa Barbara County Hazard Mitigation Plan and CalOES My Hazard (a webpage that allows you to type in your facility address and it provides you with wildfire, earthquake and flood risk). 2017 Statewide Medical & Health Exercise - November 16th – Scenario: Earthquake! Your facility does not need to conduct the full scale exercise on the November 16th but remember the new CMS rule requires one full scale exercise but November 15th. Facilities will have to participate on November 16th by submitting status reports, communicating with PHD etc. - Workshops in September- How to Conduct a Full Scale Exercise- Will be held in Santa Barbara & Santa Maria Date/Time TBD- look for more information. - Trainings in October  Santa Barbara: October 10 1-5pm; PHD  Santa Maria: October 12 1-5pm; Location TBD Flyer with all training, workshop information for the 2017 Earthquake exercise will be sent out within the next week.

The Great California Shakeout is an annual earthquake drill that will be held on October 19th at 10:19am. For more information and resources you can check out their webpage: https://www.shakeout.org/california/index.html Standing Agenda Items 1145 Partner Emergency/Disaster Plans  EMS/PHD available to review plans to assure alignment with County plans.  Use the disaster plan insert with contact phone numbers provided by PHD/EMSA  Be sure to include evacuation warning/order issued via telephone, AND reporting status and requesting assistance from PHD/EMS in your checklists. Roundtable: Participant Update on Current Activities and Needs 1150 Friendship House- State licensing visited- asked for all fire alarms to be pulled to test the system and looked at disaster binders. Vista del Monte- Conducting tabletop earthquake exercise to review current process Valle Verde Retirement Community- Concluded annual safety fair for staff and also some of the staff

2 and residents completed the Community Emergency Responders Team (CERT) training. Cottage Health- had a data outage and are planning on conducting active shooter drills this year City of Santa Barbara- Conducting CERT trainings at Valle Verde for staff and residents Hillside House- conducted evacuation exercise during night shift- evacuated 59 people in 20 minutes American Indian Health Services- conducting tabletop on August 3rd with an earthquake scenario and fill be conducting a full scale exercise on October 19th during the Great Shakeout Sansum- Training for records outage and business continuity system also working on internal radio system MRC- first aid booth at fiesta.
