CHECKLIST: L-2 CHANGING TO H-1B STATUS (Revised 11/20/2016)
From L-2 Status Holder Copies of passport identity and validity pages. Copies of all pages in current and previous passports with visa and entry/exit stamps. Copy of current I-94 record printed from https://i94.cbp.dhs.gov/I94/request.html Copies of all previous L-2 status approval notices (Form I-797), if status extended or changed in US. Photocopy of spouse’s L-1 passport identity and validity pages, I-797 approval notice, I-94 record, and three most recent pay advices or pay stubs. If ever in J-1 or J-2 status, List of all dates scholar was in J-1 or J-2 status in the US; and Copies of all DS-2019s or IAP-66s. If subject to the 212(e) 2-year home residency requirement: Evidence that scholar spent two years in home country after J time in US; or Waiver approval notice Photocopies of diplomas and transcripts of courses. Translation(s) if not in English. Evaluation of foreign credentials, if available. Updated C.V. or resume, including list of publications If involved in any patient care: Colorado medical or dental license Documentation of graduation from medical school in the US or a foreign state OR full and unrestricted license to practice medicine in a foreign state Evidence of passage of Steps 1, 2, and 3 of the U.S. MLE. (Step 3 not required if Beneficiary completed med school in U.S.) ECFMG Certificate (Canadian citizens exempt)
From Dependents to obtain H-4 status Completed Form I-539 (fillable form available from USCIS website at www.uscis.gov, ISSS can provide a sample form with helpful instructions) Copies of dependents’ passport ID and validity pages, US visas and entry stamps, any previous L-1 or L-2 approval notices (Form I-797), and I-94 records, Copies of children’s birth certificates and translations to show relationship to H1B Beneficiary. Copies of marriage license and translation to show relationship to H1B Beneficiary.

Instructions for Completing Payment Voucher Form in CU Marketplace: Supplier: Type in “US D” and select “US Department of Homeland Security.” Select “California Service Center.” Payee Type: Non-Employee Warrant Delivery Code: Select "Campus Mail" and type “DC A005/185” as campus mailbox to send directly to ISSS. Payment is being Authorized for: Choose ““Immigration/Visa Fees” from drop-down menu. Business Purpose: Type “USCIS Fees for H-1B petition on behalf of [foreign national’s first and last name].” Amount: Include the appropriate amounts from the chart below.
