Dear All 17 & 18 Year Olds- If you would like to donate blood on Tuesday, November 10 th , please complete the form below. NO WALK-IN APPOINTMENTS WILL BE ACCEPTED!!!! Your teacher must give you permission to go to the Blood Drive. All forms for donation must be turned in to Mrs. Baumgartner, Ms. Grobsky or a Key Club officer. The donation time takes anywhere from 30- 60 minutes. You are responsible for any work you may miss from class. Also, please have your parents/guardians sign below in the space provided so they are aware you are donating.
