AMTC Contestant Plan and Agreement

Between ______, hereinafter called the Contestant, and/or the Contestant’s legal guardian, and _____The Actor’s Scene______, hereinafter called the Director, to provide instruction, guidance, and representation to attend the upcoming Millie Lewis Actors, Models & Talent Competition (AMTC). All prices below are in U.S. dollars.

Director’s AMTC Package Price: $3595-Deluxe or $1535 Bare Bones Check items included in your package o Minimum of 24 Hours of Preparation, Workshops, and Training Offered Prior To AMTC. This preparation may include, but is not limited to: Runway and Stage Techniques, Wardrobe Planning, Skin Care, Natural Makeup, Hair Styling, Nutrition, Posture, Photo Movement, Public Speaking, Interview Techniques, Acting in TV Commercials, Acting in Scene Study, Monologues, On- Camera Auditioning, Performance Preparation for Singers and Dancers, Photo Selection, Self-Confidence Development, A Parent’s Role in This Industry, and more! o All AMTC Workshops On Site– a minimum of 20 hours are offered at AMTC by national experts, including the “Agent’s Panel Discussion.” o Five - Six Full Convention Days. o Enrollment in three (3) Official AMTC Competitions– choose from Runway, Photography, Swimsuit, Spokesmodel, TV Commercial, Scene Study, Monologue, Cold Reading, Improv, Singing (Popular, Country, Gospel, Broadway, Singer/ Songwriter, Group, and more), Dance, or other new events. o Exposure to Agents, Managers & Casting Directors of the highest caliber. o One–on–One Speed Networking Interviews for all Contestants and One–on–One Call Back Interviews for select Contestants with Agents in attendance. o Personalized Certificate of Participation and an Official AMTC Program Book. o Eligibility for Contracts, Trophies and Cash Prizes. o Dance party group entertainment and the Official AMTC T-shirt. o A color photo in the Official Program Book (value $45) o One 6 -Day Observer Pass. (Additional passes may be purchased for $200) Siblings 12 and under are free. o Professional Photo Session with AMTC Approved National Photographer, including Photo CD. You own your negatives. o Model’s Portfolio Case or Actor’s Headshots. Optional or Extra Expenses Not Included in this Package

 Additional Competitions beyond the first three, which are included. Extras competitions are $75.00 each.  Awards banquet ticket. (purchase not required)  Travel to the AMTC. Meals while you are there. Lodging: the Official AMTC Hotel offers a special convention rate. Please ask your AMTC Director. Contestant’s Official Refund Policy Summary Notify your Director of cancellation in writing. If written notice of convention withdrawal is made: A. Within 5 days of registration: full refund is given less a $50.00 handling and administration fee. B. 6 days – 14 days after registration, but prior to 6 weeks before AMTC starting date: full refund less a $250.00 deposit. C. 15 days – 27 days after registration, but prior to 6 weeks before AMTC starting date: 50% of total Convention fees less a $250.00 deposit. D. 28 days – 2 months after registration, but prior to 6 weeks before AMTC starting date: 25% of total Convention fees, less a $250.00 deposit. E. More than 2 months after registration or within 6 weeks of AMTC starting date: no refund is given. F. Please allow 3-4 weeks when making payment by personal check to receive a refund. G. In the event that the Contestant is unable to travel to or participate in AMTC because of weather, curtailment of transportation facilities, acts of God, terrorism, strikes, war, governmental decrees, illness, injury, or family emergencies, the Contestant is not entitled to a refund. However, the Contestant may participate in the next AMTC, a rescheduled AMTC, or any part of AMTC that was missed at no additional convention cost. AMTC is not responsible for travel expenses. H. Contestant understands that this Agreement is between the Director and the Contestant. The Director is responsible for refunds, not AMTC. Payment Plan Agreement I would like to reserve a place as a Contestant in the Millie Lewis Actors, Models, and Talent Competition. I agree to make the following payments on time. I will be given payment dates at my Orientation. If I am more than 3 days late with a payment, I agree to pay a $15.00 late fee. If I am late with the final payment, I agree to pay a $50.00 late fee, as AMTC headquarters will penalize my Director. I understand the payment plan. Actor’s Scene Customer Satisfaction Guarantee Although The Actor’s Scene and AMTC do not guarantee callbacks or agent representation, The Actor’s Scene does guarantee you will be happy and satisfied with the program. If you are not satisfied, The Actor’s Scene will take you to the very next AMTC free of charge*. Special Notes: ______
