Troup County CHIP Housing Rehabilitation Program

I. PURPOSE The Troup County Commission (County) is soliciting Requests for Qualifications (RFQ) from contractors or other agencies / organizations (herein throughout this RFQ referred to as “Contractor”), which currently perform or are qualified to perform Lead-Based Paint Inspections / Risk Assessments.

The County has received federal Community HOME Investment Program (CHIP) funds from the Georgia Department of Community Affairs to assist very low-income homeowners with making code-related repairs to their homes.

The Contractor, and / or agents of the Contractor, chosen to perform the tasks delineated under the Scope of Services must hold current certifications as a Certified Lead-Based Paint Inspector / Risk Assessor, as determined by the Georgia Department of Natural Resources, Environmental Protection Division. The chosen Contractor, and / or agents of the Contractor, must be certified to operate and XRF Analyzer.

II. SCOPE OF SERVICES The Contractor is to perform Lead-Based Paint Inspections through XRF analysis (all inspections / risk assessments shall be performed according to Federal and State requirements); produce a Hazard Control Plan with cost estimates; assist with work write-ups for abatement; and provide technical assistance to the lead abatement contractors chosen to perform lead-based paint reduction / abatement work for each dwelling unit. These services are to be provided on an “as needed” basis.

III. SPECIFIC DUTIES A. Lead based Paint Risk Assessments Perform a Lead-Based Paint Risk Assessment using XRF protocol and dust and soil sampling based on HUD and State requirements. Prepare a Risk Assessment Report and provide to the Program Administrator.

B. Lead Hazard Control Work Plan If lead hazards are found, prepare a Lead Hazard Control Work Plan with specifications and drawings and identify abatement methods to correct the lead hazards. Include cost estimates and provide to the Program Administrator.

C. Lead Abatement Inspections Perform clearance inspections according to HUD and State requirements. Provide a clearance report for every clearance inspection based on State and Federal requirements to the Program Administrator.

D. Miscellaneous Contractor will be available for one (1) walk through of inspected property with the lead abatement contractors to answer any questions regarding the Hazard Control Plan, as needed. The Program Administrator will coordinate the walk-through of the property with the homeowners and Contractors.

As needed, assist the Program Administrator in preparing a scope of work for contractors to carry out lead abatement activities listed in the Hazard Control Work Plan.

IV. CONTRACTOR PRICING Contractor shall provide a separate price for each item specified on the RFQ form, which is included with this document, based on the following format:

Price shall be inclusive of all costs incurred by the Contractor. No separate reimbursement for travel or other costs are permitted.

V. AVAILABILITY OF CONTRACTOR PERSONNEL Contractor shall specify the name(s) and experience of each person who would perform the items identified in the scope of services, herein.

VI. CONTRACTUAL AGREEMENTS The County will execute a contract for these services. The following are important provisions that will be contained in the contract.

A. Conflict of Interest The Contractor agrees to avoid all instances wherein he / she might have a potential and / or actual conflict of interest regarding his / her own personal activities.

B. Termination for Cause The County may terminate its Contract with the Contractor, for cause, upon ten (10) days prior written notice to the Contractor indicating the Contractor’s default in the performance of any term of the Contract. C. Termination for Convenience The County may terminate its Contract with the Contractor, for its convenience, at any time by written notice to the Contractor. In the event of the County’s termination of the Contract, the Contractor will be paid for those services actually performed up to the point of the termination effective date. Partially completed performance of the Contract will be compensated upon a signed statement of completion submitted by the Contractor which shall itemize each element of the performance and which, upon review and approval by the Program Administrator, will be paid.

VII. SECTION 3 AND MBE / WBE BUSINESS PREFERENCE Preference will be given to businesses that are qualified Section 3 or Minority- / Women- Owned Businesses. Preference will be given by adding an additional ten (10) points to the application scoring for Section 3 or Minority- / Women-Owned Businesses. Contractors will be required to submit documentation confirming their status as a Section 3 or Minority- / Women-Owned Business, if such status is claimed, before points will be assigned to the application.

VIII. RFQ SUBMISSION REQUIREMENTS All questions or requests for additional information should be directed to Nancy Seegar, County Planner, by email at [email protected] or you may call Nancy Seegar at 706-883- 1650. The deadline for receiving questions is 4:00pm on Monday, February 09, 2015.

All RFQs must be received at the Troup County Government Building no later than 4:00pm on Tuesday. February 17, 2015. Any RFQ received after 4:00pm will not be accepted. Whether the RFQ is sent by mail or commercial express service, the Contractor is responsible for the actual delivery of the RFQ to the County before the deadline time.

One (1) original and one (1) copy of your RFQ should be submitted. All RFQs shall be submitted in a sealed envelope / package. The envelope / package shall be addressed to Nancy Seegar, Troup County, P.O. Box 1149, LaGrange, Georgia 30241, and the following clearly printed on the outside: your company name, LEAD INSPECTION, and date.

EVALUATION FACTORS All responses to the RFQ will be evaluated to assess Contractor qualifications, experience, quality of work, ability to meet program schedules, and Section 3 or Minority- / Women- Owned Business status. Rating factors and their corresponding values are as follows: Cost of services up to 40 points Experience up to 15 points Time frame for completing tasks up to 15 points References up to 10 points Section 3 Business up to 10 points MBE / WBE Owned Business up to 10 points Total Possible Points up to 100 points

The County reserves the right to reject any and all RFQs, to waive technicalities, and to award to the lowest responsive and responsible party, as deemed in the County’s best interest. Cost of deliverables, lead-based paint experience, knowledge of HUD and CHIP guidelines, and Section 3 or Minority- / Women-Owned Business status are factors that will be considered in the award.
