Field Trip Permission Form – Caving Trip Monday, November 12, 2007 -- Colonial Forge, North Stafford &Stafford Senior & Sites

Some of the ninth grade students from the Commonwealth Governor’s School will be participating in an all day field trip on 11/12/07, or 11/13/07. Each day, part of the ninth grade CGS students will be traveling to Grand Caverns and Fountain Cave in Grottos, VA, as part of an Earth Science unit on karst topography.

The cost of this trip is $15.00. Please pay your science teacher at your CGS site NO LATER THAN FRIDAY, NOVEMBER 2, 2007. Ask your science teacher about to whom the checks are made payable. CGS will pay the cost of the bus.

FOOD - Students are asked to bring food and drink for lunch; snacks for the trip up and back; and DRINK ONLY for Dinner. Dinner will be provided! We will be ordering pizza, pepperoni and cheese, for dinner. If you do not like pizza then bring additional food for dinner. Eating is allowed on the bus. ALLERGY ALERT!! We have students that have severe allergies to nuts and nut products. DO NOT bring nuts or any nut products on the trip. This includes products like snickers & cracker products with any nuts and nut oils. If you are confused concerning this alert, talk to your teacher.

EQUIPMENT and SAFETY – Although students will be entering a non-commercial cave, the risks are minimal. There is no lighting, but some paths have minimal grading and Mr. McCloud has taken students into this cave as part of the summer regional Governor’s School for years. The teachers leading the wild cave activities, Mr. McCloud and Mr. Morris, are both accomplished cavers. There will be no rappelling, but students have the option to participate in a fairly tight crawl. Helmets and lamps will be provided, however students are asked to bring 4 fresh alkaline AA cell batteries. They should also wear stout hiking shoes or boots that may get wet. Clothing for the cave should include something OLD which are LIKELY TO GET MUDDY. Coveralls are recommended. Students should bring a complete change of clothes (including shoes and socks) to change into after their wild cave experience and a garbage bag for the muddy items. Jeans, tennis shoes, and a sweatshirt are appropriate for the commercial cave that is lighted and has gravel paths. Teacher Permission

Release of All Claims for Grand Caverns and Fountain Cave, Grottos, Virginia

I, the undersigned, hereby request permission to enter Fountain Cave, located at Grand Caverns Regional Park, Grottoes Virginia, and to participate in the exploration of Fountain Cave this day. I know the general nature of cave explorations and I know the risks and dangers involved in such activity, and that unanticipated and unexpected dangers may arise during such activities, in and about the premises.

In consideration of the permission granted to me to enter the premises and participate in the stated activity, I hereby, for myself, my heirs, administrators and assigns, release, remise and discharge the owners, operators and sponsors of the premises and activities therein and their respective servants, agents, officers and officials, and all other participants in the stated activity of and from all claims, demands, actions and causes of action of any sort, for injury sustained to my person and/or property during my presence in and around the premises and my participation in the activity due to negligence or any other fault.
