WELLSPRING AFTER SCHOOL PROGRAM GOALS PLEASE RETAIN THIS SHEET FOR YOUR RECORDS.

o Each participant will be given a short academic assessment in September or when they begin attending the program. Participants will show improvement on this assessment when it is administered at the end of the program. o The program will provide a safe, caring and supportive environment. o The staff will be positive role models and encourage participants to have good behavior by following their examples. o The staff will provide homework help and support for all activities. o Participants will be comfortable being with people of different racial, ethnic and cultural backgrounds. o Participants will solve problems without hitting, throwing a tantrum or using hurtful language. o Participants will report that they like themselves and feel that other people value them. o The program will provide materials and opportunities for parents to be involved and be informed of daily activities. Arrival and Dismissal We open at 3:00. Closing times are: On Mondays program ends at 6:00. On Tuesdays and Thursdays program ends at 7:30. On Wednesdays and Fridays program ends at 6:30. If your child is dropped off by Wellspring then you need to be home 30 minutes before the listed closing times. If you are not home 3 times when we try to drop off, then we will no longer provide transportation. Please refer to the monthly calendar for any changes.

Dress Code Please make sure your child is always appropriately dressed for the weather. We often go outside for recreational time so children will need jackets, coats, gloves, hats, etc.

No Personal Items/Electronic Devices Children should not bring electronic devices to program. This often increases the likelihood of them getting lost, stolen or damaged. In addition, children should not borrow, lend or trade items while at program. Cell phones are NOT permitted to be used at program. Children are responsible for their own personal items (coats, backpacks, etc.) The staff reserves the right to confiscate any and all personal items as issues arise. Confiscated items will be returned to the parent at the end of the program day.

Lost Items Wellspring Interfaith Social Services is NOT responsible for any personal items that may have been lost, stolen, damaged or gone missing during the program.

Transportation Limited, free transportation is provided by Wellspring. This must be arranged in advance by calling the youth department office at 422-6618, ext. 109. The staff reserves the right to refuse transportation to any child because of behavioral issues or if parents are consistently not home at drop off after program.

Photography Waiver I permit Wellspring Interfaith Social Services and its’ supporting organizations to use and publish photographs and/or videotapes of me and /or my children for purposes of promoting the Wellspring After School Program.

Behavioral/Medical Issues The staff reserves the right to send any child home that is ill or has disruptive, unsafe, destructive or abusive behavior. We may require a doctor’s note, if your child is sent home because of an illness, before your child may return to program.

Attendance Children must attend school in order to attend the after school program. If your child is ill, has been suspended, etc. then he/she may not attend program that day.

Contact Information You are expected to supply us with any address, telephone numbers, transportation changes that may occur throughout the school year.
