ITEM: REQUEST FOR PROPOSAL for Emergency Standby Generators

Proposal Date: April 12, 2016 2:45 P.M.

The following addendum is hereby made to the Request for Emergency Standby Generators:

1. The proposal date was changed by Addendum 1 issued on April 4, 2016. The correct proposal date is as stated above: April 12, 2016 at 2:45 P.M. CDT. Proposals will be opened in the Joel S. McTopy Council Chambers at 3:00 P.M. that same date.

2. The last date to ask questions on this RFP is April 7, 2016.

3. The Master Fee Schedule has been revised as follows:

Item 1000.08B was revised: Delete2/0 4 Conductor Cable; add #2 4C cable. Item 1000.08C was deleted. Item 2000.04 was revised: Delete (11) 100’ 4/0 cables; add (10) 100’ 4/0 cables. Item 2000.07 was revised: Delete (3) 75’ 4C cables; add (10 75’ 4C cable. Item 2000.08 was revised: Delete (1) 50’ 2/0 500W; add (1) 50’ #2 4C SO Cable. Item 2000.11 was revised: Change East 24th, Reserve to East 23rd, Reserve. Item 2000.12 was revised: Delete 100’ 2/0 4 conductor cables; add (4) 100’ 4/0 Cables. Item 2000.13 was revised: Delete (1) 75-100’ 4/0 cable; add (4) 75-100’ 4/0 cables. Item 2000.14 was revised: Delete (4) 75-100’ 4C cables; add (1) 75-100’ 4C #2 SO Cable. Item 3000.11B was revised: Change East 24th, Reserve to East 23rd, Reserve. 4. Under General Terms and Conditions, Article 8.0, add to the end of the paragraph, the statement in bold as follows:

8.0 Term The contract shall be for a three (3) year period beginning with execution of the contract an d ending thirty-six (36) months thereafter, with an option to renew for one (1) year increments, three (3) times upon written mutual agreement between the Parish Council and the Company. St. John the Baptist Parish shall retain the option of modifying the scope of work annually as it pertains to the number of generator sets required to be kept on standby for Parish use.

5. Exhibit A, St. John the Baptist Parish Professional Selection Committee Score Card has been revised as follows:

A. Delete:

Key Personnel Qualifications and Experience 0-10 Points Qualifications of the project manager and key personnel who will be assigned this project, including their experience on similar projects. Project Manager should have a minimum of 10 years of experience conducting sub-surface storm drain line & catch basin cleaning.

B. Change the Weight of the following Score Card Factor as shown to 0-15 Points

Key Personnel Qualifications and Experience 0-15 Points Qualifications of the project manager and key personnel who will be assigned this project, including experience on similar projects. Project Manager should have a minimum of 10 years of experience of supply and maintenance of rental generator equipment.

C. Change the Weight of the following Score Card Factor as shown to 0-15 Points

Location and Staffing 0-15 Points The location and staffing of an office from which the work will be performed. A list of staff shall be submitted, including the primary function of each key employee. The services covered by this proposal document are for the standby and installation, when required, of any/all items awarded. All materials/equipment will be on standby within a 30 mile drive to St. John the Baptist Parish Warehouse, located at 1801 W. Airline Highway, LaPlace, LA.
