DO NOT RETURN THIS FORM IF YOU ARE ALREADY OBLIGATED AFTER SCHOOL ON ANY OF THE DATES/TIMES BELOW TO: SPORTS PRACTICES, GAMES, BAND, CHEERLEADING, DOCTOR’S APPOINTMENTS, COURT DATES, WORK, OR ANY OTHER ACTIVITY. FOR THESE 12 DAYS DRIVER’S ED. IS YOUR #1 PRIORITY AND MUST COME FIRST.

Dates of class: December 2,3,4,5,6,9,10,11,12,13,16,17

If absent on Dec. 2, you WILL NOT be in the class. The class location will be announced. If late, you will be dropped! Time of classes: 2:30-5:00 pm EVERYDAY. If you are late you will be locked out. Transportation: All students must be picked up NO LATER than 5:15 pm. Any time after 5:15 pm constitutes a violation of this contract. Each student is allowed one violation in which a letter will be sent home. After the second violation, the student will be removed from the class and must pay to retake Driver’s Education. Cost of class: $55.00 You are STRONGLY ENCOURAGED to go to this website and pay online and you will be automatically registered and GUARANTEED a spot: http://jordandrivingschoolcharlotte.com/public_schools/harding.html Or you may bring cash, certified check, or money order the first day of class. No money will be accepted before class begins at 2:30. However, if removed from class for behavior, you will be banned from CMS Driver’s Education. Cost of class from a private company can range from $450-$600.

By signing below, I understand the dates and times of the Driver’s Education class. I also understand the transportation needed/expected each day for the class. Furthermore, I understand that this form DOES NOT GUARANTEE placement in the class. Bring your form to the Main Office or Student Services! Any student absent on Oct. 2 will not be placed in the class no matter what the reason for their absence. You may be dismissed from Driver’s Ed. for:  Bringing food /drink into the classroom.  Being picked up after 5:15 pm.  Vandalism of the Media Center Classroom/bathrooms.  Inappropriate/distracting behavior/profanity
