Job Title: Social Services Provider I Part Time Position number: 212936 PS:Beh Hlth/Social Worker (WL) Salary: Based on experience and credentials Location of Position: Lowndes OP C&A Apex Valdosta, GA Description of Duties: Under general supervision, provides evaluation and therapeutic services to mental health consumers in a school and community setting. Responsibilities include completion of complex psychosocial assessments, treatment team participation, treatment planning/monitoring and discharge planning. Provides individual, family and group counseling to consumers and their families. Work in community/school environment during school hours is required.

Minimum Qualifications: LCSW, LMSW, LPC, LAPC, LMFT or LAMFT with 2 years of experience OR Master’s degree in social work, psychology, or counseling which must include a supervised clinical practicum and must be registered with the Georgia Composite Board of LPCs, LCSWs and LMFTs to seek associate license. Two years of full time experience involving direct client contact such as therapy, case management, intake assessments and crisis screening.

Pre-employment drug testing and a criminal records background check are required. If appointed, applicant will be subjected to a Fingerprint Based Background Check of the Georgia and Federal Criminal History Record Databases.

Preferred Qualifications: Applicants with an LCSW, LPC, or LMSW with two or more years work experience in an MH/DD/AD office performing therapist/case management duties. Work experience within a school setting is also preferred.
