SWEETWATER MISSION (formerly known as C.A.M.P.) – 770-819-0662

Darlene Duke, Director www.svcamp.org 6289 Veterans Memorial Hwy, Bldg. 12A Austell, GA 30168 The actual, physical location is 2 blocks off of Veterans, at the corner of Hotel St. & Line St.

Food and Clothing Walk-In Service Hours are Monday through Friday 9 a.m. – 1 p.m.

Sweetwater has no specific zip code residency requirements. Anyone who qualifies for assistance, based on income, can receive food, no matter where he or she lives.

Some food comes from the USDA’s TEFAP distribution, so income levels are important. All household incomes must be counted. “Household” includes everyone living under one roof, regardless of relationships. Clients are required to sign a Federal Declaration of Income Statement.

The table below shows monthly and weekly income for each family size. Households at or below the income listed for the number of people in the household are eligible to receive food.

Household Monthly Weekly Size Income Income 1 $1,211 $ 279 2 $1,640 $ 378 3 $2,069 $ 477 4 $2,498 $ 576 5 $2,927 $ 675

(For larger households, see actual form)

Under this program, food can be distributed to a household with just a minimum of 30 days between visits. 8 CSB clients can be served in any one of three ways.

1. CSB clients who attend The Circle program - A client can turn in a food request form to The Circle’s receptionist, and a staff member picks up the food and takes it back for the client. The food can be taken home on the van. Clients receiving food in this way do not receive any frozen or refrigerated food. They only receive a box of canned goods and staples. Only clients who have no transportation and who have not registered at Sweetwater (formerly C.A.M.P.) on their own can request assistance through The Circle in this way. 2. Dual Diagnosis clients – Dual Staff will take the completed form to Sweetwater to pick up the food. 3. CSB clients can go as walk-ins on their own, as anyone in the community can do. These clients would receive refrigerated and frozen food, as well as a box of canned goods and staples. Clients must know the income for everyone in the household. It is helpful to have a photo ID with them also. Clients who go on their own cannot also request food through The Circle’s program with Sweetwater. 4. Case managers can pick up food on behalf of clients. In this instance, our agency’s ROI form would need to be completed. Sweetwater has provided us with a special form to use when requesting food in this way. There is a place for the client to sign giving a CSB Representative permission to pick up his or her food. This method would necessitate that the staff member wait for service, along with other people in the community. It is possible for a staff member to pick up food for several clients, as long as all of them have completed the form giving income information and giving permission for the staff member to pick up the food.

Sweetwater also offers other services. Their regular clients can shop for clothing, hygiene items, or household goods and can also be eligible for special gift program invitations. In order to qualify for these extra services, it is necessary to have some form of ID for every household member seeking assistance. A driver’s license, state ID, social security card, tax ID, birth certificate, voter registration, or a school or immunization record can be accepted.
