Notification of Student Failure Teachers: No student may receive a failing grade unless: a) You have the specified minimum number of grades to average for the marking period; b) You have provided some type of intervention(s) to the individual student; c) You can document twice having informed the parent directly (face-to-face, on the phone, note, or interim report) that the student will fail: and d) Principal has been notified with copies of the documentation attached. Directions: Please create a separate form for each student that received a failing grade for this marking period or cumulatively as a “Final Grade” for this school year.
