CIRM Biographical Sketches for Key Personnel and Letters of Collaboration and Institutional Support Use this template to complete Part C of the Targeted Clinical Development Awards application, the Biographical Sketches for Key Personnel (including the clinical investigators) except for technical staff and students. All biographical sketches should be assembled into a single document observing the two-page limit per individual. We recommend that you convert the final document into a PDF file to ensure that formatting is preserved when submitting. Append any letters of collaboration and institutional support to the end of the biographical sketches PDF document.

Provide the following information for the PI, Co-PI (if applicable), and Key Personnel participating in the project, including the lead clinical investigator at each clinical site. Highlight experience and activities relevant to this proposal. To add an additional biographical sketch, select the template pages, copy and paste to create new pages.

1. SELECTED ACCOMPLISHMENTS/ HONORS List major accomplishments and honors in chronological order beginning with most recent. Applicants may include accomplishments such as projects managed, IND submissions etc.

2. EXPERTISE AND RELEVANT EXPERIENCE Provide a description of prior experience relevant to the proposed role in the project.

3. SELECTED PEER-REVIEWED PUBLICATIONS/ PATENT APPLICATIONS/PATENTS (in chronological order). Do not include manuscripts submitted or in preparation.
