An animal adopted from HK Rescue Puppies (HKRP) is free, however we do ask for adopters to please pay for the animal’s medical costs incurred so far (e.g. injections, worming tablets, Frontline, medication, check ups, vets tests, microchip, desexing etc.), plus costs for their food, treats, homing bag, transportation etc. & (if possible) to make an additional donation to HKRP. In return for giving you the above-mentioned animal for adoption, we ask that you please agree to the following:

 I shall be a responsible pet owner and take good care of the animal from the moment of signing this form to the end of his natural life.  All training will be done by kindness, love and reward, NEVER by threat or punishment.  I shall take him to a qualified veterinary surgeon for all advised regular check ups, vaccinations and when the animal is sick or injured. In addition I shall give the animal the necessary periodic treatment (vet recommended) for the prevention of stomach worms, heart worms, ticks and fleas.  I understand that HKRP, while undertaking to give accurate information to the best of their ability, cannot make any guaranteed claims as to the animal’s health, temperament, exact age or history and shall not be liable for any existing or hidden injuries and/ or diseases or behavioural defects suffered by the animal. The costs of any and all treatment (if any) required for the animal will be borne by me.  If the animal is not already de-sexed, I shall have him de-sexed by a qualified veterinary surgeon between 5 & 6 months of age (de-sexing is compulsory BEFORE sexual maturity).  I shall inform HKRP at once if problems arise and allow HKRP to help (if possible).  I do not work in the pet trade industry & on no account will I intentionally kill, maim, eat or sell the animal or allow it to be used for breeding or experimental purposes.  I shall not transfer ownership of the animal or give the animal to any other person or organisation without first informing HKRP and obtaining their prior approval.  By completing and signing this Animal Adoption Agreement, I fully indemnify HKRP against any present or future claims regarding the animal in question.  I understand that, should I fail to honour any part of this Agreement or if I neglect or ill treat the animal in any way, he/ she may be reclaimed by HK Rescue Puppies and all rights of ownership will be revoked.  The information I have provided is truthful and accurate.  I understand and accept the contents of this Agreement.
