Position Summary:

The Community Health Field Nurse, LPN, provides nursing care which requires the full range of nursing proficiency in clinic and home setting under the direction of the Community Health Nurse, RN/Supervisor. This includes assessing the needs of patients with a wide variety of conditions, setting up medications, operating special equipment, and responding to urgent situations in both individual and group settings. The nurse must recognize and respond appropriately to patient conditions that deviate from normal and identify and report on causative factors and reactions.

Duties and Responsibilities:  Carries out community health plans by conducting visits to client homes, the Senior Citizen Center, the Tribal Health Center, or elsewhere on the Reno-Sparks Indian Colony where patient care services are needed.  Interviews patients and family members to determine and evaluate health and physical needs. Consults with or refers to the medical provider or RN as needed.  Reviews findings to distinguish between normal and abnormal conditions during patient encounters.  Carries out treatments, sets up medications, observes and documents patient condition, changes, and reactions to medications and medical treatment.  Provides information and education to patients, families, or surrogates on prevention, curative, and rehabilitative measures involved in health care in both individual and group settings.  Maintains client health records by documenting observations, nursing interventions, and therapeutic measures administered.  Maintains daily log of activities and clients and provides a monthly report to supervisor.  Assist with patient transportation, as needed, for medical appointments and deliver pharmaceuticals to patients who do not have access to the clinic due to lack of transportation.  Maintains knowledge and proficiency in nursing through continuing education, staff meetings, and department in-services.  Performs other duties as assigned. Minimum Qualifications:  Graduation from an accredited college or university with a Baccalaureate Degree in Nursing, current license to practice as a practical nurse in the State of Nevada. One (1) year work experience as a Community Health Field Nurse or as a Home Health Nurse.  Knowledge of principles and practices of nursing as applied to community health practice, preventive medicine, and sanitation.  Proven experience to work effectively with individuals and/or families to assist them in satisfactory solution(s) to health relationships with others.  Confirmed ability to effectively communicate both verbally, and in writing to various individuals, cultures and groups.  Ability to meet the physical demands of walking, standing and some lifting during the performance of normal duties.  Must possess a valid Nevada Driver License and ability to qualify for Reno-Sparks Indian Colony vehicle insurance.  Must have current CPR certification.  Working knowledge with experience documenting in electronic health record or other similar programs.  Must pass TB skin test.  Must be able to pass pre-employment alcohol & drug screen.  Must pass PL 1001-630 Indian Child Protection & Family Violence Act background check.  Must be able to work in a diverse minority environment.  Excellent customer service skills a must.
