Attached, please find the quotation for the MCG Servo Brush motor with Brake. This custom motor has a part number of CYM3395. The model number is ID42001 with a 15 in.-lb. internal brake. The output shaft would be 0.625 Diameter. It will include MS Connectors for the Motor and Tachometer and a separate one for the Brake.

The standard delivery is eight weeks after the receipt of the order.

 Prices do not include state or local taxes.

 Cymatix, Inc. will strive to meet all requested due dates, however, Cymatix, Inc. is not responsible for delays in delivery when such delays are due to causes beyond our control.

 All discrepancies applicable to shipments must be reported within 10 days of receipt of goods.

 Merchandise returns cannot be made without an RMA number from Cymatix. A minimum of 20% handling/restocking charge will apply to all authorized returns. Goods must be received in new condition.

 Custom and/or non-standard items are not returnable.

 Terms of payment shown are subject to credit approval.

 Clerical and stenographic errors are subject to correction.

 All shipping and handling charges are the responsibility of the customer.
