For parents with more than one child please complete a booking form for each child.  We offer a 10% discount for NHS Staff, please ensure you send us copy of your id badge or payslip. NHS discount is only applied to the first child, standard sibling rate will be applied to all other children. Please tick if you qualify for the NHS discount.

Parents signature______Date______TERMS AND CONDITIONS FOR PARENTS/CARERS Spring Meadows Holiday club runs during all school holidays and some days over christmas, (excluding non-pupil days,) from 7.50- 6:15pm. We are based at The Scout Hut, Barclay School, Walkern Road. Stevenage. SG1 3RB We have fully competent and trained staff that will endeavour to provide suitable activities for all the children who attend. The staffing ratio is 1 adult to 8 children under 8 years and 1:10 for children over 8 years. The children are provided with refreshments; which includes drinks and a light hot/cold snack (which they help to prepare). Please bring a packed lunch for your child.

In the event of an emergency, the club can be contacted on the club's mobile 07522 960627  All children must have a upto date online child registration form completed.  Children can be dropped off at Holiday club from 7.50am.  Children must be collected from holiday club by 6:15pm prompt, or before.  Staff need to be informed if someone other than the parent/guardian will be collecting their child. If that person is not known to staff then they will need to provide a "password", which will be told to the parent/guardian when they advise us that somebody else will be collecting the child.  All children must be registered with us before they can attend Holiday club and all relevant forms must be completed by parent/guardian.  All fees are payable by the end of the month by either Bacs or Childcare vouchers o Bacs Account Name Spring Meadows, Account name 01486672, Sort Code 30-99-31.  All booked sessions must be paid for, even if you do not attend.  Any sessions your child will not be attending should be notified to [email protected]  Persistent failure to notify us of sessions your child is not attending may result in contract termination.  A charge will be made for late collection; which is £5 for the first 15mins and then £5 for each 15mins or part of thereafter per child.  Any sensitive information exchanged by any person involved with Holiday Club must be kept strictly confidential. CONTRACT OF AGREEMENT

The parent(s) of any child/children attending Holiday club should have read and understood the "Terms and Conditions" before signing the following declaration: 1) Having read, understood and accepted all statements made in the "Terms and Conditions", I would like my/our child/children to be accepted into the club. 2) I/We understand that any decisions made by the organisers will be final. 3) I/We agree to pay the session fee by Childcare voucher or bacs. 4) I/We will ensure all relevant forms have been completed and signed as necessary. 5) I/We understand that the clubs run from 7.50 to 6:15pm and that there will be a charge for late pick - ups. 6) I/We undertake to inform the staff if someone other than the parent/guardian is to collect my/our child/children. 7) I/We understand that all absence will still be charged for. 8) I /we understand all additional sessions must be booked through [email protected] and can only be booked if space is available. 9) I/ we understand failure to notify Abi of a cancelled session may result in contract termination. 10) I /we understand if an all day trip is planned I must book my child in for a full day. 11) I /We understand trips are an additional cost.
