POSITION: FAMILY SERVICES COORDINATOR

Supervisor: Part B Special Education Coordinator General Responsibility: Responsible for coordinating all components of an early childhood program that serves children birth through age five. Minimum Educational/Certification Qualifications: B.A., B.S. degree in social work or a related field.

Experience: Experience or training in social work or case management is preferred. Essential Functions: (The following functions will be carried out under mentorship of an Early Childhood Special Educator)

1. Must be able to interact in a positive and effective manner with a wide variety of people including children, parents, families, staff members, and community members in order to promote positive relationships within the work setting and community. 2. Must be able to meet time lines for required program documentation such as (but not limited to) case note documentation, child outcome summary forms, assessment reports as well as any required reports related to the job, child goals, objectives, etc. and manage multiple priorities associated with the requirements of the job while maintaining an effective and efficient work schedule. 3. Must have the ability to gain knowledge and adhere to the Rules and Regulations Governing Services to Children with Disabilities according to the Individuals with Disabilities Education Act (IDEA). 4. Must be reliable, organized and detail oriented. 5. Must possess extensive computer experience and ability. 6. Serve as initial contact for families and facilitate family interviews. 7. Process referrals for early intervention services and establish a Multi-Disciplinary Assessment Team or Screening Team. 8. Must be able to provide necessary documentation of MDAT reports in a clear, concise, accurate, and timely fashion. 9. Must facilitate and participate in the development, review and evaluation of all IFSP (Individualized Family Service Plan) or IEP (Individualized Education Plan) meetings to ensure compliance. 10. Assist families in developing strengths and skills necessary to support children with disabilities or delays. 11. Develop and maintain interagency contacts along with making appropriate referrals to other community or regional agencies. 12. Monitor master files and assure that all required forms are completed. 13. Assure that all IFSP and IEP services are provided as stated. 14. Coordinate all transition activities to another program of the parents/guardians’ choice. 15. Facilitate the development of individual transition plans from Part C to Part B programs and from Part B to Kindergarten. 16. Facilitate communication between the family, IFSP/IEP team and other service providers. 17. Facilitate problem solving and collaboration of team members around changing needs. 18. Support the facilitation and coordination of the Parent Education Council. Essential Functions (cont’d):

19. Support and attend community agency meetings as necessary. 20. Coordinate the enrollment of children into the preschool program. 21. Ensure the safety of all enrolled children and families participating in CDC programs. 22. Must follow all policies and procedures of the Child Development Center. 23. Must be able to attend required staff meetings and trainings. 24. Perform other duties as assigned.

Physical/Environmental Requirements:

1. Must be able to work in a variety of positions, including but not limited to: stooping, kneeling, crouching, crawling, reaching, sitting, etc. 2. Must be able to lift 35 lbs. unassisted. 3. Must be able to tolerate air conditioned buildings and fluorescent lights. 4. Must be able to work effectively in a stressful environment. 5. Must be able to hear and process ordinary conversation, telephone conversation, and quiet sounds with or without aid. 6. Must be able and willing to drive to meet job requirements. 7. Must be able to operate and use equipment necessary to perform essential functions of the job.
