Czech Republic Trade Mission October 3-5, 2005 Sponsored by Food Export USA-Northeast, Mid-America International Agri-Trade Council (MIATCO) and the Michigan Department of Agriculture PARTICIPATION AGREEMENT

Thank you for your interest in the Czech Republic Trade Mission. Below you’ll find the terms and conditions of this activity accompanied by a registration form. To register, simply read the information below and return the completed form and registration fee to Food Export USA-Northeast.

Registration Fee: Affiliation with Northeast or Midwest Region *Non Affiliation with Northeast or Midwest Region Before Aug. 12, 2005 $475 Registration Deadline Before Aug. 12, 2005 $712 Registration Deadline

*Out of Region companies will be admitted on a space available basis

Participation Terms and Conditions Food Export USA/MIATCO agree to : 1. Create a mission catalogue which will include the company profiles of each participating company. 2. Conduct one-on-one meetings, a table top showcase, and a reception with key retailers, importers and distributors. 3. Provide a seminar explaining the Czech Republic food market and labeling requirements. 4. Provide an import analysis and competitive store check for one of your company’s products ($350 for each additional product). 5. Provide interpreters during the mission. 6. Provide guided tours of major supermarket chains in Prague.

Participating Company agrees to : ( Please initial each statement ) 1. Pay participation fee and submit this completed Participation Agreement no later than the Registration Deadline of August 12, 2005. 2. This participation fee does not include flights, hotel expenses, meals and the shipping of samples to the Food Export USA office. Participants who do not attend will be billed. _____ 3. Promote only those products manufactured in the US with a minimum of 50 percent US-origin agricultural content, by weight. _____ 6. Indemnify Food Export USA, MIATCO and the Michigan Dept. of Agriculture from any financial loss or other claims resulting from participation in the Czech Republic Trade Mission. _____ 7. Upon completion of this activity, complete an evaluation and provide information regarding increased export sales in this target market or otherwise resulting from the activity and an estimate of your company’s financial contributions incurred while participating in this activity. ____ 2
