Metromont is committed to helping you improve your health and supports a culture of wellness. Our Wellness Program is available to both you and your spouse (if enrolled in Metromont’s medical plan) through Vitality. Vitality is an active, fully-integrated global wellness program designed to engage, encourage, and reward you on your personal pathway to better health. By participating in Vitality, you can earn a lower premium on your health insurance and Vitality Bucks to spend in the online Vitality Mall. This rewards program allows you to redeem your hard-earned Vitality Bucks on items such as fitness devices, iTunes and Amazon gift cards, movie tickets, and so much more.

Getting started is easy – simply go to https://www.powerofvitality.com and click on the “Register Today” link. You will need your employee number to create your account. Your spouse (if applicable) will also need to create his/her own account and will use your employee number to sign up. IMPORTANT: If your employee number begins with “0”, you need only to enter the last 4 digits of the number.

Once you and your spouse (if applicable) have created your accounts, each of you must:

1. Complete the Vitality Health Review (VHR). The VHR is a brief confidential survey in which you are asked about your current health status, activity level, and nutrition habits. It is also the first step in earning your Vitality Points. Your answers help to create a personalized health summary, so the more honest you answer these questions, the more accurate your feedback will be.

2. Complete the Vitality Check (VC). The VC is a biometric screening that includes lab work. There are several ways to complete this: . Visit the Onsite Health Clinic for your Clinical Health Assessment (CHA). You will receive your CHA results from TargetCare and must manually upload the information on a Vitality Check Form through the Vitality website. Upon receiving your results, you are encouraged to review your information with a TargetCare health coach. NOTE: This option is available only to associates who work at a location where there is an Onsite Health Clinic; spouses may not use this option. . Schedule an appointment with your personal physician. Take the Vitality Check Form to your doctor’s appointment and then manually upload the information through the Vitality website. . Metromont’s annual biometric screening event. This option is available to new associates hired between 10/1 – 12/31. Information on the annual screening event will be announced later in the year. . Quest – You may schedule a Wellness Screening appointment at a Quest Diagnostic Patient Service Center (PSC). There is no cost to you for this screening. Please refer to the “Quest Scheduling Instructions” at the end of this overview for information on how to schedule an appointment.

2017 Vitality Wellness Program Overview Vitality Age From your answers on the VHR, you are assigned a Vitality Age, which is a health risk-related age based on your lifestyle behaviors and clinical measures, and is a calculation of your overall health and well-being relative to your actual age. It is intended to inform and motivate you to being active and participating in healthy lifestyle activities.

Vitality Reward Status Your Vitality Reward Status is a great way to track your progress and stay motivated. It’s simple: As you earn more Vitality Points, you increase your Reward Status level. You begin at the Bronze level by taking the VHR and VC. As you accrue Vitality Points, you increase your Reward Status from Bronze to Silver to Gold to Platinum, the highest Vitality Reward Status available through the program.

Vitality Points and Vitality Bucks Vitality rewards your efforts to improve or maintain a healthy lifestyle. When you engage in health-related activities that can be tracked and measured, you earn Vitality Points. There are a wide variety of Vitality activities designed to empower you with the tools necessary to reach your optimal health. Take online nutrition courses, set your own health goals, exercise at a gym, use a fitness device linked to Vitality, or have regular medical screenings. There are many options, and it’s recommend that you engage in as many as you can.

In addition to increasing your Vitality Status with Vitality Points, you may also use those same points to reward yourself by earning Vitality Bucks. Every Vitality Point you earn equals one Vitality Buck. When you increase your Vitality status, you are rewarded with Bonus Bucks. Vitality Bucks can be redeemed at the online Vitality Mall located on the Rewards page on the Vitality website.

Vitality HealthyFood The Vitality HealthyFood Program is a partnership program designed to reward you for making healthy choices when purchasing food. Not only will you save money when you make healthy choices at the grocery store, but you can also earn Vitality Points. There are two ways to earn points through the program – with a Wal-Mart HealthyFood Card or through the NutriSavings Program and your bonus card from your local participating grocery store. Click on the HealthyFood link under the Manage Your Links section on the Vitality website.

Vitality Squares “Find the Fruit – Win Some Loot”! Vitality Squares is a fun and easy way to earn points. Once you and your spouse (if applicable) have completed your VHR, you each can play up to once a month. The higher your Rewards Status goes, the more squares you get to turn over and the greater chance of winning you have. Vitality Squares is located under the Rewards section of the Vitality website. Metromont Incentive Plan The amount you pay for your medical insurance is linked to your Vitality Status. A standard Vitality year is January 1st to December 31st. If you are hired after 1/1/17, you have until 12/31/18 to achieve Gold status. In the meantime, you will automatically receive the Gold premium rate. If you have not achieved Gold status by 12/31/18, you will lose the premium incentive and pay the premium of the status level you are at that time. Each year thereafter, your Vitality status at the end of the year will determine your premium for the following year.

When you complete your VHR and achieve Silver status, you earn a $15 subsidy towards the purchase of a fitness device through the Vitality Mall. You are also eligible to receive a contribution to your Health Savings Account from Metromont based on the following achievements:  $200 = achieve Gold status  $200 = achieve Platinum status

Note: For associates in the Hourly pay group who are voluntarily participating in the Vitality Program, your HSA contribution is earned through compliance with the TargetCare “Right-on-Target” Wellness Program. Please refer to the Wellness Program Overview for Hourly Associates for more information.

Spousal Engagement If your spouse is enrolled in Metromont’s medical insurance, he/she also participates in Vitality. Both you and your spouse receive points which are combined to earn Vitality Bucks and to achieve a higher Vitality Reward Status.

Medical Waiver If you or your spouse has a condition that limits/prevents you from participating in physical activity or from achieving standard in-range medical recommendations for biometric results, or if you are pregnant, Vitality can provide a Reasonable Alternative Standard (RAS) as an accommodation. To qualify for the RAS, you/your spouse must complete a Waiver Form and submit it, along with a signed letter from your physician attesting to your condition, to Vitality.
