2014 Scholarship Application

Eligibility: 1. Be a High School Senior graduating in the Spring of 2014. 2. The applicant must be an active member of The Luke for at least one (1) year. 3. Be active in at least one (1) ministry in the church. 4. Submit a letter of recommendation from Servant Leader of the ministry in which the applicant actively participates (provided in an envelope sealed by the leader). 5. Submit a copy of the official high school transcript indicating high school completion and cumulative GPA of a minimum of 2.5. 6. Have attended Next Level College Prep event(s) sponsored by Fusion Youth Ministry (please provide the date of the College Prep event you attended and a 1 paragraph essay about how the event enhance your higher education pursuit)

Note: Applications that do not meet each of the above 6 criteria will not be considered.

Requirements: 1. The Pastor of Student Ministries must receive the completed application and all required attachments no later than the close of business (5pm) Wednesday, June 11, 2014. 2. All applicants meeting the requirements will be invited to the next phase: a personal interview. The interview must be completed by Thursday, July 10 2014. 3. Scholarships recipients will be notified no later than Monday, July 21, 2014. 4. Submit a copy of 1) your acceptance letter and 2) your registration receipt or tuition bill. 5. Scholarship awards will be released upon receipt of proof of acceptance and registration to a 4-year college or university, 2-year college, or technical school.

Award: 1. The award is a one-time (non-renewable) award and will be paid in two (2) increments of $500 (1 each on or about August 15th for the Fall semester and on or about January 15th for the Spring Semester) for a total of $1,000 for the first year. 2. All awards will be sent directly to the financial services department of the school.
