Consent form for participants in Substance Leadership and social roles among the Chabu of Southwest Ethiopia.

We’re interested in different aspects of leadership. Ed Hagen is the study leader and I, Zachary Garfield, will be working on it. We want to learn about what types people are leaders, the costs and benefits of leadership, and the things leaders do for the group.

I will ask you about what people generally think about leaders and what types of qualities are re- quired to become a leader. I will ask you about what types of activities require a leader. I am also interested in learning what the differences are between bad leaders and good leaders. In all, topics will cover demographic information, ideas about leaders, and you may be asked to com- plete a peer rating activity about personality traits of individuals in the community. Also, we may measure your height, weight, and waist and hip dimensions. In total, the questions and proce- dures should take about 45 minutes to complete.

Ed Hagen and I will keep the answers that you give us confidential. We will not use your name in any of the materials we publish or present to the public.

Some people may be uncomfortable discussing these topics or rating other individuals. If you experience any feelings of discomfort during the interview, you can skip the question or withdrawal from the study without penalty.

I [Zachary Garfield] will answer any questions you have about what we’re doing. The project has been reviewed and approved by the WSU Internal Review Board.

You don’t have to join this project and if you do, you can quit at any time. There will be no penalties or bad feelings if you don’t want to do this.

If you are willing to participate, we will pay you [20 Birr/$1 USD] at the completion of the interview.

Please inform the translator if you consent to participate and give your permission for them to sign indicating your consent.
