Please quote your lowest price on the Procurement of Analog FXO Media Gateway, subject to the conditions also listed below, stating the shortest time of delivery and submit your sealed quotation which is duly signed by your representative not later than 10:00 A.M. of 8 January 2013 to the following address:

NOTE: 1. As a government agency, LWUA shall deal only with legitimate suppliers/contractors which issue BIR-registered official receipts. 2. All entries shall be typewritten. 3. Quoted prices shall be inclusive of applicable taxes and shall be firm and valid for a period of at least thirty (30) days from the date of receipt of quotation & shall be binding upon the supplier within the period. 4. Delivery period shall be thirty (30) calendar days from receipt of Notice of Award/Purchase Order. 5. Warranty shall be for a period specified in the Technical Specification. 6. LWUA reserves the right to post-qualify any supplier and/or to reject any or all submitted quotations. 7. The P.O. shall be awarded to the lowest calculated responsive bid; delivered items are subject to inspection, with payment processing to commence only after acceptance by the end-users. 8. Valid PhilGEPS Registration Certificate shall be submitted with the quotation. 9. Terms of payment within 30 calendar days from date of acceptance. 10. Approved Budget for the Contract P 290,000.00. 11. Request for Quotation may be purchased by interested Bidders starting 17 December 2012, 9:00 A.M.- 4:00 P.M., and upon payment of nonrefundable fee in the amount of Five Hundred Pesos (P500.00) in the form of cash to the LWUA Cashier.
