Chapter 5 Chapter 5 Endometriosis and PCOS: Two major pathologies… Endometriosis and PCOS: Two major pathologies linked to oxidative stress in women Sajal Gupta1, Avi Harlev1 and Ashok Agarwal1 Introduction Oxidative stress (OS) ensues when the detrimental activity of reactive oxygen species (ROS) prevails over that of anti-oxidants causing lipid peroxidation, protein carbonylation, and DNA damage and/or cell apoptosis. Moreover, reactive nitrogen species (RNS), such as nitrogen oxide (NO) with an unpaired electron, is also highly reactive and toxic (Agarwal et al. 2012, Doshi et al. 2012). OS has been known to participate in the pathogenesis of PCOS and endometriosis. Several OS biomarkers have been scrutinized by investigators, in the past, including MDA (malondialdehyde), protein carbonyl, TAC (total antioxidant capacity), SOD (superoxide dismutase), GPx (glutathione peroxidase), and GSH (reduced glutathione) to determine the role of OS in PCOS (Azziz et al.) and endometriosis (Jackson et al. 2005, Murri et al. 2013). Free radicals are known to impact several microenvironments in different biological windows, such as in the follicular microenvironment (Gonzalez et al. 2006, Murri et al. 2013). Both PCOS and endometriosis are associated with poor oocyte quality and infertility (Gupta et al. 2008, Goud et al. 2014, Huang et al. 2015). Our current review addresses the role of OS in both these disease conditions and the role of antioxidants and lifestyle modifications in preempting the impact of free radicals in PCOS and endometriosis. Methodology: Trans Abdominal Ultrasonography was carried out over 157 women in a rural medical college of West Bengal and 51 females were diagnosed of PCOD using Rotterdam’s criteria. Results: Maximum prevalence of PCOD was seen between 15 to 24 years age group. Dominantly oligomenorrhea was seen among PCOD (75%) patients. 33.4% obese patients were diagnosed PCOD. Conclusion: It is commonly observed in early child bearing age group, especially those females having oligomenorrhea. Lifestyle management is now considered one of the principal way to deal with PCOS. Keywords: Anovulation, B.M.I. Born in 1901 in Turin, Italy, his time on earth was short-only 24 years-but he filled it passionately with holy living. Pier Giorgio was Born: April 6, 1901, Turin, a model of virtue, a "man of the beatitudes," as Pope John Paul II called him at Italy the saint's beatification ceremony in Rome on May 20, 1990. Strictly defined, menopause is the last menstrual period. It defines the end of a woman’s reproductive years as her ovaries run out of eggs. Now the cells in the ovary are producing less and less hormones and menstruation eventually stops. What is perimenopause? On average, the perimenopause can last one to four years. It is the period of time preceding and just after the menopause itself. In industrialized countries, the median age of onset of the perimenopause is 47.5 years. However, this is highly variable. It is important to note that menopause itself occurs on average at age 51 and can occur between ages 45 to 55. Actually the time to one’s last menstrual period is defined as the perimenopausal transition. Often the transition can even last longer, five to seven years. What hormonal changes occur during the perimenopause? When a woman cycles, she produces two major hormones, Estrogen and Progesterone. Both of these hormones come from the cells surrounding the eggs. Estrogen is needed for the uterine lining to grow and Progesterone is produced when the egg is released at ovulation. -

Diagnosed with PCOS, was on metformin for 2 years. Self discontinuation. Seen by gynecologist • Progesterone withdrawal – positive. OCP’s – intolerance (weight gain, headache). Denies galactorrhea. Has some facial hair (upper lips) – no change since teenage years. No neurological symptoms, weight changes, fatigue, HTN, DM-2. • Currently – plans for conception. • Pertinent P/E – BMI: 24 kg/m², BP= 120/66 mm Hg. Fine vellus hair (upper lips/side burns). Methods: The current study was a cross sectional study carried out in Assiut Women's Health Hospital between the 1st October 2016 and 31st July 2017. The patients were selected as infertile patients with PCOD. The patient ages range between 20 and 35 years. The BMI is between 18 and 30 Kg/m2.
