2012 Silent Auction Shipping & Donation Form

Are you shipping your Silent Auction donation? Please use this form.

If you are hand-delivering your donation, you do not need to fill out this form. Simply fill out the Onsite Donation Form at the drop-off area when you deliver your donation.

Thank you for supporting the Glass Art Society by donating! Your donation to the GAS Auction helps keep conference registration fees affordable and supports low student fees and ongoing operations of GAS. You can make a full donation or choose to receive 10%, 25% or 40% of the sale price. As an auction donor, your name will be listed in the 2012 GAS Journal.

Delivery: We encourage you to bring your donation with you to the conference.

To Deliver in Person: Please hand-deliver work packed well enough for Bring to the Waynesefield Room: carryout. If you are delivering your art by hand, you do not need to fill out Wednesday June 13 noon – 5 pm the form below. Donations will be accepted in the Waynesfield Room at Thursday June 14 9 am – noon the Park Inn at the following times: Friday June 15 9 am – noon

To Ship: If you are unable to attend the conference or cannot hand- Ship to (must be rec’d by June 6): deliver your item, you may ship donations (at your own expense). Schmidt/Messenger Studio Please include this form (one form per donated item) in your shipment. 340 Morris Street Shipped work must be received in Toledo by June 6. Please insure your Toledo, OH 43604 package and indicate that it must be signed for. ATTN: Glass Art Society — Auction

Additional Information:

1. The silent auction occurs on Saturday, June 16, 2012, at the Park Inn in Toledo.

2. If your donation does not sell, please reclaim it immediately after the live auction ends, at 7 pm. Unsold works of art that are not reclaimed by their seller at the end of the auction event become unrestricted property of GAS. GAS shall be entitled in our absolute discretion to exercise disposal of the work as we deem appropriate after this date. 3. Shortly after the conference, you will receive a letter including the sale price of your piece, the name and address of the purchaser, and (if applicable) a check. 4. GAS promises to handle your piece(s) with the utmost respect and care, but we are not responsible or liable for breakage, theft, or loss.
