POSITION DESCRIPTION
TITLE: Staff Chaplain
DEPARTMENT: Pastoral Care
DATE: 8/1998, 6/2003, 4/2008, 11/2014, 8/2015 FLSA STATUS: Exempt
NATURE AND SCOPE OF POSITION
Provides spiritual support to all patients, their families, friends, Associates, and volunteers, of all denominations, faith beliefs, and ages. Pastoral care chaplains provide a spiritual connection with their community churches.
PRINCIPAL RESPONSIBILITIES
To provide spiritual support within Centegra Health System to all patients, family members, friends and staff in all situations and circumstances with a special focus on Holistic care for the Mind, Body, and Spirit. Focusing on the Centrga Health System Mission, Vision, and Values and conforming to all HIPAA regulations.
Serve as member of the interdisciplinary clinical team to provide emotional and spiritual support. Visit newly admitted patients daily, complete assessments of patient’s spiritual needs. Communicate with all local clergy and their congregations regarding the spiritual needs of the hospital patients as requested by the patient. Request sacramental services as needed from community clergy. Provide spiritual care information through the interdisciplinary patient charting process as directed by Centegra policy. Attend Interdisciplinary Patient Care discharge rounds, lead spirituality groups, facilitate Goals of Care conversations, advise and complete Advance Medical Directives with patients and families.
Availability for flexible scheduling of work hours.
Demonstrates knowledge of the principles of growth and development over the life span, and possess the ability to assess information reflective of the patient’s spiritual needs. Includes being able to interpret the appropriate information needed to identify each patient’s requirements relative to the patient’s religious beliefs and their age-specific needs. Identification of population served: Infant to older adult.
Demonstrates knowledge of and daily practical application of Centegra’s Service Excellence Standards and Customer Satisfaction objectives.

Participates in the departmental annual planning, departmental policy-making, and interdepartmental planning efforts.
Follows a regimen of personal and professional growth through continuing education and attending related conferences and seminars.
Provides service as a resource through education and consultation within Centegra Health System and the community clergy and churches.
This position adheres to and is responsible for the safety standards of Centegra Health System including the completion of the mandatory organizational safety standards on an annual basis.
This position actively maintains confidentiality for our patients and their families as outlined in the CHS Patient Confidentiality Policy and shows the same level of respect for every Associate with in CHS.
This position performs other duties as assigned.
MINIMUM KNOWLEDGE, SKILLS and ABILITIES - REQUIRED
One unit of Clinical Pastoral Education or equivalent.
Bachelor’s degree in Theology or related field.
Must be in good standing with his/her faith organization.
Strong interpersonal skills with the ability of self-direction.
OTHER PREFERRED
Two to Four units of Clinical Pastoral Education.
Master’s degree in Theology or related field.
Two years hospital related experience.
Bi-lingual, English and Spanish preferred.
Board Certification.
