The ultimate goal of this program being twofold: allow employers to retain and grow their existing Ohio workforce and create a statewide workforce that can meet the present, and future, demands in an ever changing economy. This employer-driven program is designed to provide direct financial assistance to train workers and improve the economic competitiveness of Ohio’s employers. The voucher program is designed to offset a portion of the employer’s costs to upgrade the skills of its incumbent workforce and will provide reimbursement to eligible employers for specific training costs accrued during training. The program’s funding will be used in conjunction with private contribution to fund skill-upgrade training. Page Name Ohio Shared Services/Vendor Number Registration Links (left) Page copy Ohio Shared Services In order to receive reimbursement payments through the Ohio Incumbent Worker Training http://ohiosharedservices.ohio.gov Voucher Program, employers must have an established Vendor ID Number with the State of Ohio. Employers who do not have a Vendor ID Number should visit the Ohio Shared Services (OSS) website at http://ohiosharedservices.ohio.gov and follow these steps to NOTE: I would like the OSS website register: to open up in a new window when the link is clicked on, so the user  Click on “Vendor” (link on the left, third picture down). This will direct employers to the required forms for providing information to OSS.  Under the heading “Ohio Shared Services Vendor Forms, Click on “Dear State of Ohio Potential Vendor Letter”. This letter directs the employer to complete three forms in order to register as a vendor to do business with the State of Ohio and thus receive a Vendor ID Number.

The required forms are:  Vendor Information Form (OBM-5657-Rev.11/1/2011)  IRS Form W-9 Request for Taxpayer Identification Number & Certification  Authorization Agreement for Direct Deposit of EFT Payments (OBM- 4310-Rev.11/1/2011)
