NOT PROTECTIVELY MARKED Rev 08/15 Page 1 of 4 TEMPORARY STAFF APPLICATION FORM GUIDANCE NOTES Thank you for your interest in temporary work with Warwickshire Police (WP) and West Mercia Police (WMP). Please read this guidance document before completing your application.

1. ELIGIBILITY CRITERIA Nationality Applications can only be accepted from British citizens, European Economic Area (EEA) nationals, Commonwealth Citizens or Foreign Nationals with no restrictions on their stay in the UK.

As part of our recruitment process we need to authenticate your identity and check that you are entitled to work in the United Kingdom (UK) in order for us to comply with the terms of the Asylum and Immigration Act 1996.

If you are a Commonwealth Citizen, Foreign National or non-UK British citizen you should include a copy of your passport which shows your stay is free of restrictions. Other documentary evidence of your status may be required.

Residency Criteria To ensure that satisfactory vetting checks can be completed, applicants to the Police Service are required to have been resident in the UK for at least three years immediately prior to their application. For posts with higher level vetting requirements, this period is extended to five years.

Applicants who have been serving abroad in the British armed forces or on UK Government service are considered to have been resident in the UK.

2. COMPLETING THE APPLICATION FORM

Please provide all previous names and/or surnames/family names.

Give the town, county and country of your birth.

2. Contact Details

Please provide an email address which we can use to update you on your application and be clear about which telephone number(s) you prefer to be contacted on.

3. Relationships

If you are related to, or have a close personal relationship with anyone working for WP/WMP please provide their name, status within the organisation (if known) and relationship to you. The declaration of any such relationship will not disqualify you from consideration; however, failure to disclose any such relationship or to directly/indirectly seek the support of any employee (canvassing) for any appointment within WP/WMP may lead to rejection of application or may be dealt with under the appropriate procedure which may include the Disciplinary Procedure.

Any applicant who is unsure whether or not a relationship should be declared is advised to contact the Recruitment Team.

4. Qualifications and Training

Please ensure that you provide information about all relevant examinations, training and qualifications you have undertaken. Include any qualification you are currently studying for or any job specific training you have received with past employers (including in-house training events) which are relevant to this post.

Please ensure that you provide details of the grade date that the qualification was obtained. If you are invited to interview, we will ask you to provide original qualification certificates.

5. Driving Qualifications

This information will only be used in short listing where the essential criteria require the post holder to drive a vehicle.

Please provide details of your employment history to cover at least the last 10 years, starting with your current, or most recent, employment and working backwards. Include any periods of self-employment. If you have been a full-time student, please provide the names and dates of the establishment that you attended. Include the title of the course you attended in the 'post title' section.

Include any relevant previous experience whether paid or unpaid, and details of any periods in which you have been unemployed or in education. If you have been unemployed, or in education, please nominate an individual who can provide a reference for this period. Personal references cannot be given by family members, police staff or police officers.

You may continue on a separate page, if necessary, in order to provide the required history.

References will be requested from your previous two employers (including your current employer, if applicable) or from your school or university, to cover a minimum of three years. References from your current employer will not be taken up until you have given us permission for them to be approached. Please ensure that your nominated referees are aware of your application, so that they are able to supply the required information in a timely manner.

7. Skills Profile The information you provide will be submitted to the recruiting manager to assess whether or not you meet the criteria for the role. Please provide as much detail as possible to ensure you can be considered for as many different types of role as possible. Please do not send a curriculum vitae (CV).

10. Cautions and Convictions

You must declare if you have ever been investigated, arrested, summonsed, charged, cautioned or convicted for any offence by a UK or non-UK police force, law enforcement agency or any other statutory prosecuting authority or agency - this includes, but is not limited to:

 traffic offences (including fixed penalty notices excluding parking);

 receipt of an absolute/conditional discharge or bindover;

Page 3 of 4  receipt of a reprimand, warning, final warning or caution as an adult or juvenile;

 being the subject of an Anti-Social Behaviour Order, Football Spectator Banning Order, Risk of Sexual Harm Order and/or Harassment Order;

 being issued with a Penalty Notice for Disorder or other Fixed Penalty Notice (other than parking).

11. Declaration

Once you have fully completed the application form please read, complete and date the declaration to agree that the information you have provided in the application is true to the best of your knowledge and belief and that you have not withheld any relevant information.
