POSITION TITLE: Smokefree ABC Engagement

REPORTS TO (Title): CDHB Smokefree Manager

PRINCIPAL OBJECTIVES

. To contribute effectively to the work of the Smokefree ABC team, which includes:-  Working collaboratively with CDHB staff, PHOs and local community organisations towards reducing the harm from tobacco use and exposure to environmental tobacco smoke  Supporting health professionals in primary and secondary care to effectively deliver the ABC Strategy for Smoking Cessation  Supporting the CDHB staff and PHOs to achieve the targets set in primary and secondary care and maternity for Health Target 5: Better Support for Smokers to Quit  Providing ABC training and support, as appropriate, to health professionals, key staff in workplaces and organisations and community and health workers.  Supporting Maori, community and statutory organisations to create sustainable ABC initiatives using a community development approach  To ensure awareness and utilisation of referral pathways to Te Hā – Waitaha Stop Smoking service  Supporting the CDHB to effectively implement the CDHB Smokefree Policy . To work co-operatively as a Smokefree ABC team member . To maintain links with local cessation providers and promote referral pathways . Timely evaluation and progress reports are made available as required . Appropriate workforce development shall be undertaken

FUNCTIONAL RELATIONSHIPS: (Who are the customer/consumers/patients) Internally: 1 CDHB clinicians, managers and other staff 2 CDHB patients who smoke 3 Community and Public Health health promoters 4 Te Hā – Waitaha staff 1. Externally: Smokefree Support May 2017 1 PHO and CDHB Secondary Care staff 2 General Practice teams 3 NGOs, social service agencies, workplaces and community organisations 4 Primary care organisations, including pharmacy 5 Te Hā – Waitaha provider organisations 2. Smokefree Canterbury

KEY PERFORMANCE OBJECTIVES:

Task Work collaboratively with the CDHB Smokefree Manager and the Smokefree ABC team to ensure the DHB Smokefree contract and the Ministry of Health service specifications are effectively delivered in primary and secondary care and in the community. Expected Result  A collaborative relationship with the ABC team members is established and maintained  Smokefree champions in a range of settings are identified and provided with appropriate support and training  Referral pathways to cessation providers are effective and well utilised  Effective information channels are used to disseminate new information as appropriate  Staff who smoke are provided with support to manage their nicotine dependence and stop smoking Task Facilitate effective hospital and general practice systems to support staff to deliver the ABC Strategy for Smoking Cessation and achieve Health Target 5: Better Help for Smokers by:  identifying the smoking status of all patients  providing a brief intervention to those who smoke  providing ready access to Nicotine Replacement Therapy  offering support to quit and a referral to a cessation service Expected Result  ABC systems in primary and secondary care are regularly reviewed to facilitate the efficient delivery of ABC  All patients who smoke will be identified, provided with a brief intervention and an offer of support to quit  People who smoke have easy access to Nicotine Replacement Therapy or other cessation medications  Health Target 5 is achieved  Increased numbers of referrals to Te Hā - Waitaha Task ABC training is provided to staff in primary and secondary care, workers in social service organisations, workplaces and the community appropriate to their roles, functions and positions Expected Result  Workers in the CDHB, primary care and the community understand and deliver the ABC effectively and confidently  Resources, training and information updates are provided regularly and appropriately to those delivering the ABC and / or smoking cessation Task Be accountable for providing evaluation and progress reports as per pre- determined schedules and as requested Expected Result  Ongoing liaison with CDHB Smokefree Manager  Requested information is provided within the required timeframe(s)  Related evaluations/surveys/audits to affirm practice and monitor effectiveness are carried out within the expected timeframe Task Responsible for undertaking appropriate personal and professional development

Smokefree Support May 2017 Expected Result  Personal development and education opportunities undertaken as recommended, and as deemed appropriate by manager  Current skills are maintained and updated as required  Annual performance appraisal undertaken Task Responsible for following all relevant CDHB policies and procedures Expected Result  Relevant legislation, regulations, policies and procedures are adhered to

HEALTH & SAFETY:

 Observe all Canterbury DHB safe work procedures and instructions  Ensure your own safety and that of others  Report any hazards or potential hazard immediately  Use all protective equipment and wear protective clothing provided  Make unsafe work situations safe or, if they cannot, inform your supervisor or manager  Co-operate with the monitoring of workplace hazards and employees health  Ensure that all accidents or incidents are promptly reported to your manager  Report early any pain or discomfort  Take an active role in the Canterbury DHB’s rehabilitation plan, to ensure an early and durable return to work  Seek advice from your manager if you are unsure of any work practice

QUALITY:

Every staff member within CDHB is responsible for ensuring a quality service is provided in theIr area of expertise. All staff are to be involved in quality activities and should identify areas of improvement. All staff are to be familiar with and apply the appropriate organisational and divisional policies and procedures.

QUALIFICATIONS & EXPERIENCE:

Essential  A health related tertiary or vocational qualification  Experience and/or an interest in tobacco control  Ability to build and maintain effective relationships  Excellent communication skills  An understanding of the concept and practice of reducing inequalities

Desirable  Smoking cessation experience  Knowledge of the health sector  Experience working with one or more of the priority groups (Maori, Pacific, pregnant, mental health, youth)
