The following is a list of items that our accounting team needs from you before you will receive an Alt number for a new employee.

1) Employee Data Sheet from Posi with the following information. A) First & last name (should be the same as the name on their banking information) B) Employee type C) Mailing address D) Postal code E) Phone number F) Date of birth G) Sex H) Date Hired (First Day Worked) I) Starting hourly wage

2) Photo copy of THEIR SIN Card, all staff should be able to provide you with their card. If their SIN starts with a “9” you must verify the expiry date. Social Insurance Numbers that begin with a "9" are issued to temporary workers who are neither Canadian citizens nor permanent residents. These SINs are temporary and valid only until the expiry date printed on the front of the card.

b) To obtain a confirmation of your SIN, or to amend your SIN record, gather all the required original proof of identity documents and take them to the nearest Service Canada point of service. If everything is in order, you will get your SIN during your visit.

c) If an employee is unable to visit Service Canada please cc the employee data sheet and all required documents which includes government issued ID (Work Visa, Passport, Driver’s License etc…) to [email protected] for approval. 3) Void check from an active bank account that they wish to have their check deposited in, if they don’t use checks they will need a stamped direct deposit form from their banking institution. If they don’t have an account they will need to open one before their first shift. The void check must have their name and address printed on it, if they do not have checks they need to obtain documentation from their bank

4) All three items, employee data sheet, direct deposit information and photocopy of SIN card, should be scanned and emailed to head office, nothing else should be sent. Please do not email this information to Violet, review your current payroll contact for head office, email them and cc. [email protected]

5) Employee E-mail information: Once the new employee has handed in their tax forms and filled out their Employee Information Sheet please sit down at your office computer and have them log into their account and email head office. In the subject line it should have the words “New Employee, their full name, and store location”

6) If an employee does not have an Alt number, they should not be working!
