Description The Children's Integrated Services Nurse Home Visitor is responsible for providing high quality comprehensive, community-based nursing services for prenatal and postpartum women, and infants and children ages birth to five in the context of their families. The nurse home visitor is responsible for maintaining the highest standards in maternal child and family health clinical nursing practice, including safe, effective nursing care based on sound scientific principles and the nursing process. The nurse home visitor will adhere to policies of nursing practice, Children's Integrated Services, and the Orange County Parent Child Center. The Children's Integrated Services (CIS) maternal child and family health nurse home visitor is responsible for case management and on-going evaluation of medical, educational, psychological, social/emotional and economic or other needs of all assigned clients. The nurse home visitor will seek and encourage input from CIS team members and other community providers in determining the priority health care needs of the maternal child and family health population; will implement prevention, health promotion, and other identified strategies to achieve population outcomes. This is a 24 hour per week, benefited position.

Requirements Two years recent experience in community maternal child and family health, public health, or mental/behavioral nursing, with skills in one or more of the following areas: assessment and evaluation, pregnancy and early child development, cultural competence, home visiting experience, life skills and extensive knowledge of community resources. Ability to work collaboratively as a member of a multidisciplinary team; ability to maintain professional boundaries with clients; excellent written and verbal communication skills; ability to be organized and maintain complete, accurate records and file reports in a timely manner. Willingness to meet regularly with supervisor to develop a reflective practice, continuous assessment of their clinical nursing skills, and identification of practice areas that need special attention.

Education Vermont nursing license. BSN preferred or ADN plus relevant experience.
