Below is a list of awards with deadlines that are presented annually to students, faculty, staff, and friends for their outstanding accomplishments or service to the school and community. Please nominate individuals you deem deserving of an award by the deadlines listed. Nominee(s) full name(s) should be as it appears in the Office of Enrollment Services and complete mailing address should be included in your correspondence for timely delivery. If the nominee is not a student, please present their names and titles as it should appear on the award.

Please do not inform the candidates until they are officially notified by the Dean. The Department Chairs will be copied on the notification letters.

SHRP RECOGNITION AWARDS

NOTE: 1. Nominees may not have received the award being nominated within the last 5 years unless stated otherwise. 2. If the deadline falls on a non-working day, the deadline will be moved to the first working day thereafter.

Distinguished Service Deadline: March 1

This award is presented by the School to an individual or organization. SHRP’s adjunct (less than 50% FTE) and volunteer faculty members may be nominated. No other SHRP employees are eligible to be nominated for this award. The award will be presented on nomination by a faculty member and the recommendation and approval of the Executive Council to an individual who has made significant contributions to the School and to health care through education, service or research. - 1 - Dean’s Citation Deadline: March 1

This award is presented to those who have exhibited outstanding contributions to the School, resulting in improved programs or services to students and/or community and who have demonstrated constructive involvement in educational or health care initiatives. Volunteer and per diem Faculty, alumni, members of program advisory committees and other members of Rutgers University or other entities are eligible. This award is limited to no more than six.

Stanley S. Bergen Medal of Excellence Deadline: March 1

The student must be a prior year's graduate and named the winner of the Clinical, Academic or the Student Excellence in Service Award. Each program may nominate one student with the approval of the Program Director. The final GPA must be 3.5 or better to be eligible for consideration. Please accompany your letter with a copy of the final transcript. Please address five (5) of the seven (7) categories listed below.

1. Excellence: to include consistent outstanding performance where applicable. 2. Leadership: to include such areas as elected positions within and outside of SHRP, leaders of group activities or projects, teaching/instructional activities, etc. 3. Scholarly Activity: to include performance in research and clinical projects, reports, presentations, papers, participation in activities such as the annual SHRP Research Day, etc. 4. Community Service: to include volunteer or community service activities in any area. 5. Professional Involvement: to include membership and attendance at professional meetings, involvement in professional committees or other related activities. 6. Service to the University: to include participation on University or school committees or task forces, or other related activities. 7. Special Projects

Note: The letter must address ONLY those activities that occurred while the individual was a student at RUTGERS UNIVERSITY in the professional phase of the program. Distinguished Alumnus Deadline: March 1

This award is presented to an individual who have made an outstanding contribution to health care service, education or research. Recipients will be chosen for their ability to work with patients and fellow professionals, their level of community involvement and for a positive reputation among health care professionals and community members. I urge faculty members to help identify such alumnus or alumna. Address the required documents to the attention of Dr. Othel Rolle, Alumni President. Submit to: Mr. Douglas Lomonaco

Faculty Excellence in Service Deadline: March 1

Nominations for this award may come from peers, administrators or self-nomination and is awarded to those who have contributed outstanding service to the community, the School, the University, or Allied Health Professions.

Nominations should be accompanied by a description of the service(s) provided, a description of the impact of the service, and letters of support that verify the service(s) provided.

The following criteria must be met.

1. Service is defined as voluntary activity provided to a recognized community institution or professional organization; service to SHRP or the University beyond the assignments recognized by the Committee on Committees; provision of community programs, whether educational or direct patient/student services; or individual volunteering in the community at large.

2. Evidence of the impact made through service, particularly as it might reflect on Rutgers Biomedical and Health Sciences (RBHS), may be through letters of support or recognition, other awards received for the same service, annual reports, documentation of products or services provided, or other supplementary materials. Employee of the Year Deadline: March 1

This award is presented to a full time staff with at least three years of continuous employment with SHRP and must be in good standing with the University. The Employee Recognition Committee will select an employee based on exceptional contributions to RUTGERS UNIVERSITY - SHRP, fellow employees, students, and the community. Student: Academic & Clinical Excellence (Form on next page) Deadline: February 1-March 20

These awards are presented on the recommendation of program directors or faculty members to students in approved programs (not tracks) who have excelled academically and/or clinically in their respective educational programs. Programs without significant clinical education pay give two academic awards. A nomination form provided by Academic Affairs is attached and must be completed.

Initial nominations are due as soon as available. Final nominations are due no later than March 20th. (Note: The Office of Academic Affairs establishes the deadline for these awards. Failure to meet this deadline may result in omission of the students’ name from the Commencement Program as an award winner.).

Nominations for this award may come from peers, administrators or self-nomination and is awarded to those who have contributed outstanding service to the community, the School, the University, or Allied Health Professions.

3. Students may only receive this award once within a program of study.

4. This SHRP award is an optional award for any program.

Programs will determine criteria for awardees that are consistent with the above criteria and that are appropriate to the length of their programs. Unlike the Academic and Clinical Awards, this is an optional award.
