Event/Activity: SCOUTStrong Navy SEAL Fitness Award

I, the undersigned request voluntary participation for myself (or my minor child) in the event or activity sponsored by Boy Scouts of America and:

Am aware that the activity in which I am (or my minor child is) about to participate is potentially hazardous. I am (or my minor child is voluntarily participating in these activities and understand that my (or my minor child’s) participation may involve risk or serious injury or death, including losses which may result from my (or my minor child’s) own actions, inactions, or negligence; the actions, inactions, or negligence of others; the condition of the facilities, equipment, or areas where the event or activity is being conducted; and the ‘rules of play’ associated with this type of even or activity. I understand that should I (or my minor child) have any risk concerns, I should discuss my concerns and associated risk before I sign this document;

Consent to the collection of personal information by the Boy Scouts of America and the United States Navy for internal use as well as for displaying the event or activity results online;

Consent for the Boys Scouts of America and the United States Navy to film or photograph me (or my minor child) while participating in the event or activity and to use my (or my minor child’s) imagine, likeness, and/or voice in Boy Scouts of America and United States Navy publications, online content, video, or any other medium for their office purposes;

Certify that I am (or my minor child) in good health and have (has) no physical condition that would prevent participation in this event or activity.

Release and Consent

By signing below and in consideration for allowing my (or my minor child’s) participation in the event or activity, I herby acknowledge these risks and assume them as part of my participation. I further release from liability and agree to hold harmless the United States Navy, its officers, officials, employees, and agents from my and all claims for injuries or damage that may arise from participation in this event or activity. This agreement is intended to be a broad and inclusive as is permitted under the law. Additionally, if any portion of this agreement is held invalid, the balance shall, notwithstanding, continue in full legal force and effect.
