Purpose of Job To be responsible for the development and day to day co-ordination of clinical services in Nottinghamshire. Under the direction of the Lead Nurse and Services Manager, the post-holder will provide leadership, clinical supervision and direction to a team of healthcare staff within the specified remit. To provide support and operationally develop the services within the role’s scope.
The post holder will deliver Health & Wellbeing interventions, to service users in the Nottinghamshire area. The post-holder will contribute to the service to ensure that service users consistently receive high quality interventions that adhere to national guidance, NMC practice guidelines and which achieve high professional standards.
They will work within agreed treatment and recovery plans to support service users to detoxify from alcohol and other substances and will undertake duties involved in the day to day operation, management and development of alcohol detoxification, also assuring integrated care pathways are in place for service users who require in-patient detoxification, and/or onwards referral for chronic or acute alcohol related health needs.
The post holder will undertake comprehensive health and wellbeing assessments for service users and engage in health promotion and appropriate prevention and harm reduction strategies and interventions in order to; address and reduce physical and mental/emotional health related harms associated with drug & alcohol use. They will also help to combat blood borne virus and other infectious diseases by implementing an immunisation and vaccination scheme whilst providing professional advice and information to others in the management and care of individuals with drug and alcohol issues.
To work with stakeholders in promoting CGL clinical services.
To provide key people management support to the Services Manager and lead nurse as required, alongside the supervision of the health care team.
To ensure that all clinical process and procedures are in line with CGL policy and that all staff are aware of key policies relating to clinical safety.
To be involved in clinical audit of the service under the direction of the Clinical services coordinator.
The post-holder will contribute to the service to ensure that substance users consistently receive high quality services that adhere to the best practice guidelines and achieve high professional standards.
They will be responsible for contributing to the overall performance of the service to ensure that contractual output targets are achieved.
The post holder will be required to work flexibly within an agreed number of hours to maintain the most appropriate level of service provision. This may include some evening and weekend working. The post holder will be expected to take responsibility for personal development, identifying personal training needs, and participate in regular supervision and appraisal and ensuring that Nurse revalidation is adhered to.
Areas of Responsibility:
Management Accountability Post holder reports to the lead nurse who will provide operational direct line management and clinical supervision.
Significant Working Relationships The Senior Nurse will be expected to support healthcare staff employed in the service, via delegated line management structures. Clinical supervision will be provided by the lead nurse on a monthly basis.
Key Accountabilities:
Provide overall support and direction on developments within the health & wellbeing provision.
Provide line management, supervision and leadership to Health and Wellbeing staff.
Support the lead nurse, clinical services coordinator and deputy director of nursing to ensure that all clinical services are delivered to a high standard in line with national guidance, CGL policies, procedures and best practice, and in line with CQC Essential Standards. Ensure that procedures are constantly reviewed to promote best practice and evidenced based practice Take part and conduct clinical audits as required on services, under the direction of the Clinical services coordinator or Deputy Director of Nursing.
To manage the clinical aspects of implementations of new clinical developments within the service as required.
Work in partnership with stakeholders and promote CGL clinical service to commissioners and health and wellbeing boards alongside the lead nurse.
Ensure all healthcare provisions are adequately staffed.
Ensure self and health care staff participate in the core training programme as and when required.
To undertake regular performance reviews and identify areas of risk and development
Attend partnership meetings, to ensure appropriate representation of CGL service. Assist in the development and implementation of clinical standard operational policies so that services are effectively delivered, controlled, monitored and reported.
Ensure that all legislative requirements are explained to employees in particular the legislation governing the clinical and social care/community field.
Ensure CGL’s corporate clinical strategy and identity are reflected by all area services, both within and outside the organisation
Developing therapeutic alliances with people who have substance misuse problems and developing effective partnerships with service users to support their individual recovery journeys, engaging in key work sessions, offering health promotion and harm reduction advice and screening (including blood borne viruses) alongside other clinical and psychosocial interventions.
To undertake specialised assessments for service users, where appropriate and ensure a wide range of harm reduction interventions are made available. Utilise a range of evidence based assessment tools to assess levels of problematic drug & alcohol use and develop recovery plans based on suitability for community or inpatient detox. Ensure adherence to identified pathways for blood sample labelling, collection, obtaining results and signposting. Use blood results as motivational tools and to assess current health status, the need for specialist medical referral and the suitability for community detoxification. Conduct assessments of physical dependency using a symptomatic withdrawal scale, using results to determine safe detoxification as required. Where indicated, to refer for specialist inpatient detoxification and to liaise with hospital and inpatient unit staff. Work alongside the designated Lead Clinician and/or Nurse Prescriber to formulate, plan and implement individual community detoxification programmes to ensure safety and optimise outcomes. This will include supporting service users’ detoxification from substances across operational sites.
To offer education, support and testing for Hepatitis A, B, C and HIV providing support when delivering test results and smooth linkage and clear pathways into treatment as required. To undertake duties involved in the day to day operation of the drugs & alcohol detoxification programme service. Offer wound care assessment and basic wound dressing to clients. Organise signposting to other services as required, and ensure clear pathways into other treatment arenas as appropriate. Adhere to Patient Group Directions and ensure requirements on updating are maintained Pro-actively maintain an up to date knowledge of medicine management and both local and national guidelines and agreements. Ensure the smooth transition of service user’s clinical needs (including medicines management) into primary care/GP shared care. Ensure that all data requirements in relation to health & wellbeing targets are met within defined deadlines. Engage proactively with both line management and clinical supervision and maintain a current knowledge of appropriate interventions. Provide support to staff working with regards to needle and syringe provision. Work closely and proactively with the management and staff team to ensure adherence to all relevant Health & Safety Polices and ensure that risk is managed appropriately within the service. As required develop and provide health education especially in regard to problematic alcohol use and alcohol detoxification. The senior nurse will ensure that National & Local Health Promotion campaigns are promoted throughout service. Undertake any other appropriate tasks that may be allocated by the Services Manager, Lead nurse, Clinical Services Coordinator, Deputy Director of Nursing & Clinical Practise or the Clinical Services Director from time to time, for the benefit of services in particular geographical area as well as CGL in general. Person Specification The following are elements of the post which are deemed to be the essential or desirable qualities, experience and skills necessary to carry out the duties of the post.
Essential criteria:
First level registered nurse with a current active NMC registration Experience of managing and supervising people/teams in the clinical services, drug and alcohol or social care fields
A clear understanding of the need for and ability to deliver quality services Sound and demonstrable knowledge of clinical prescribing issues. Sound and demonstrable knowledge of harm reduction and health promotion interventions. Demonstrable experience and commitment to supporting and working within diversity awareness environments
Knowledge of the issues facing substance misusers including appropriate medical interventions, social care and health issues. Ability to assess and recovery plan service users and formulate written reports as necessary Maintain professional boundaries and work effectively with partnership agencies. Demonstrate skills to undertake administrative duties. Experience of contributing to project development.
Desirable Criteria Access to own transport (non-essential). Knowledge of local services and geography.
