Kindergarten students must be five years old on or before August 31, 2013
Pre- K students must be four years old on or before August 31, 2013 to apply
. Kindergarten students are required by law to have a Health Assessment (Physical Examination) completed by the time school starts. The required Health Assessment form can be picked up from the school office or online at www.robeson.k12.nc.us/kha
. Immunizations and Health Assessments will be offered by the Robeson County Child Health Plus Bus at some Kindergarten Registrations. You must bring the child’s Medicaid/ Insurance information to use this service. Call your child’s school (Insert Phone Number) to find out if the Health Plus Bus will be at your school site.

Student Enrollment Information The following documents are required for appropriate placement in grade level and program services:
Student Enrollment Form (will be completed at time of registration) Original Certified copy of student’s birth certificate -No hospital, souvenir or photocopies accepted Legal Guardianship Documentation for Student Current Proof of Robeson County residency, including community district Safe School Enrollment Declaration (will be completed at time of registration) If applicable, Parent/Court-Appointed Custodian Affidavit or Orders Current Immunization record -Complete record required by 1st day of school entry (must meet North Carolina requirements) IEP-A copy of your student’s (Individualized Education Plan) if applicable. Because the IEP does expire, please provide a copy of the current IEP RtI (Response to Intervention/Instruction TIERS) or 504 Accommodation Plan if applicable – Please bring documentation regarding RtI from the school you are transferring from and/or because the 504 is updated yearly, submit the current 504 plan. If applicable: Individual Health Care Plan, Emergency Action Plan, Diet Order, Crisis Plan, Behavior Plan, Custody Orders (School staff will initiate use of Student Support Checklist – SSMT) Health Assessments for all new Pre-K and Kindergarten students -Required by 1st day of school entry (must be dated within 12 months prior to school entry) A copy of a withdrawal form, most recent report card, or school transcript if applicable All applicants must submit at least one document from each of the following columns. These documents are for address verification and must all reflect the address for enrollment or change of address COLUMN A COLUMN B COLUMN C Copy of Deed OR record of most A utility bill or work order dated within Valid North Carolina driver’s recent mortgage payment the past 30 days, including: license Copy of Lease (including Housing Gas bill Current vehicle registration Authority and HUD leases) AND Water bill Valid North Carolina photo record of most recent rent payment Electric bill identification care HUD Closing Statement Telephone bill Dated within the past year: Residency Affidavit from landlord Cable bill W-2 form affirming tenancy AND record of Vehicle tax bill most recent rent payment Property tax bill Rental Contract including signature Dated within the past 60 days: page Medicaid Card Section 8 agreement Payroll stub Letter from approved agency (group Bank or credit card statement home purposes only)
Special Notes: *Legal guardianship requires additional lawful and authorized documentation from a court or agency This residency policy does not apply to homeless students, as defined by the McKinney-Vento Act. Group homes are required to provide proofs from Columns A & B only. In compliance with North Carolina law, students must be 5 years of age on or before August 31 to be considered for Kindergarten. Students applying for Pre-K programs must be 4 years of age on or before August 31.
Due Process Notice: If a student is identified as an EC student or has an IEP or a 504 Plan the EC committee or SSMT must meet to plan for placement, services and/or procedures. If this is not completed upon enrollment, provide a meeting alternative date within five days upon the enrollment process to complete this. Meeting Date ______
