Healthcare consumers are consumed with increasing cost and lack of information. The brand new CashDoctor® Mobile is a community where consumers share their own healthcare experiences, healthcare service prices and qualities, and healthcare provider reviews. Healthcare providers also play a critical role in the community by sharing their service details and prices. By using this app and sharing information with each other, our users will take control of the healthcare price in their own community for their own benefit, and thus making the healthcare prices more transparent. Both consumers and providers will benefit from a much healthier healthcare industry. Appendix 1.2: System Requirements CashDoctor Mobile only requires a smart phone running iOS or Android and a connection to the Internet. 1.2.1 Hardware Requirements  Mobile device with Android or iOS operating system. 1.2.2 Software Requirements The recommended systems for CashDoctor is:  Android 4.0 or higher,  iOS 7 or higher. 1.2.3 Other Requirements

 Device must have an internet connection, either through Wi-Fi or through Cellular Data. c.2. Installation Procedures The application can be downloaded from App store for iOS platform and Play store for Android platform. Appendix 2.1: Initialization procedures 1. Ensure that device is properly connected with the internet. And the system should have at least available 20MB space for CashDoctor. 2. Download application from online app store that is compatible with your platform.

Appendix 2.2: Re-installation 1. Uninstall the application by following the steps in section 2.3 2. Re-install the application starting from initialization by following the steps in section 2.1

Appendix 2.3: De-installation For iOS device: 1. Press and hold on application icon until the ‘X’ mark appear on top-right corner of the icon. 2. Press ‘X’ and then press OK button to remove the application and all its data. For Android device: Users can use uninstaller application or follow these steps 1. Go to device’s setting, choose menu Apps. 2. Select CashDoctor. 3. Press uninstall button. c.3. Operational Procedures After completing the installation, user can open the application by press on the application’s icon.

Login: This is the first page that comes up when you press the app icon on the phone for the first time. You can sign up for an account or login if you already has an account. If you login, you will be taken to the “dashboard”. If you press “sign up now”, you will see the register page.

Register: After pressing “Sign Up Now” on Login page, you are taken to this Register page where you have an option to register either as a consumer or a provider. As a consumer, you will be able to search for provider with specialities and location, share your healthcare experience, rate the provider’s service, post the provider’s review, or just check the recent updates of providers in your network. As a provider, you are required to register with detailed information of your business for your potential consumers. You can post changes of your services so that your consumers will see them. You can also search for other providers like a consumer will do.

Register as a consumer: To become a consumer of the community of CashDoctor, you should provide with a unique user name and email address. Also you are required to input your real name and password to set up your account. We will keep you real name and password as secured information and will out reveal them to others without your agreement.

Register as a provider: To set up your account as a provider, you have more details to fill in: real name, email, business name and address, etc. These information is provided to consumers so that they can look you up and add you into their network.

Forgot Password: This page comes up when you press on “Forgot Password” on the Initial page. You can reclaim your account and password by entering your email address. An email containing the link to reset your password will be sent to you.

Dashboard: After logging in, Dashboard comes up. If you are logging in for the first time, it will show a tutorial like the left picture shows. You will see the latest updates of the providers in your network.

Sidebar: This sidebar is accessible from every page after logging in. It takes you to Dashboard, your Profile page, your network page, and search page where you can find a provider by its location and speciality.

Edit Profile: Pressing “Edit Profile” on the sidebar, you will see this page coming up. It shows your current detailed background information in our database. You can edit your name, email, address, and even upload a photo for your profile. It is important that you keep your profile up- to-date.

Network: Pressing “My Network” from the sidebar, you will be taken to the Network page. You can see all the providers you have followed. To add more providers into your network, you should first search for the provider, go to the provider’s profile page and the press the “follow” button there.

Search: Pressing “search” button on sidebar, dashboard or network pages can take you to this Search page. You can search for a provider based on location, the provider’s name or his/her speciality. You can get your location automatically or pressing the “get location” button. Once the location is set, you can also change the location by manually input the location you want. You have an option of 5, 10, or 20 mile radius to locate the provider. If the provider is not found, you should expand the radius and attempt again.

Search Results: This page comes up to display your search results. You can view the list of providers that fit your search limitations. You can go to the provider’s profile page by pressing its photo. Or, you can just pressing its email or phone number to contact the provider. If you want to share a healthcare experience that’s related to the provider, just press “share invoice” button.

Add a Provider: If your provider is not found on the list, it means the provider is not yet in our database. You can add this provider following the link on the Search Result page. It takes you to this page where you can fill in the details of the provider. It’s important that the provider accounts in database should be valid. So, after your submission of the new provider, we have system administrator to verify the information you fill in, which is why your provider won’t be able to show up in the search results immediately. It is also important for you to fill in valid information about the provider. Thus, this page has strict validation mechanism. Please be patient to fill the information.

Share: Following the link on provider’s profile or search results, you can visit this page to share your healthcare experience with the provider. You can provide with invoices by manually input the price, service name, etc. If you want, you can also take a snapshot of the invoice. After the invoice form, you can rate the service by giving stars to different questions. These information will be posted on the provider’s profile page so that any users can find it and use it.

Provider Profile: If a user press on the provider’s name or photo that comes up in the search results, this page comes up. You can follow or unfollow the provider, which adds or removes the provider to/from your network. You can also add a service rating by pressing “post”, or review the provider by pressing “review”.

Service Reviews: On the same Provider Profile page, all the ratings and invoices shared by others will be listed below in a table. You can click every row of the table to find the details of the review on a pop up pane. If the you click on the logout option in the sidebar, or a long time has passed after your login, your session will expire and you won’t be able to see this page again until you login.

Appendix 4.1: Frequently Asked questions 1. Why should I use CashDoctor? Cash doc is a convenient and easy way to find and learn about doctors in your area. You can seek reviews, prices, service ratings, doctor specialities, etc, in this community. By sharing your own healthcare experience, you can help others to find the healthcare service that they want.

2. Should I register as a provider or as a consumer? If you are a doctor who want to share your services and have a connection with your consumers, you should register as a provider. If you are a person seeking healthcare information, you should register as a consumer.

3. If I cannot find my doctor in the community, what should I do? You can add your doctor as a healthcare provider to the community. There is a “adding a provider” link on the page of search results. You should click that link and it will bring you to the page where you can fill in your doctor’s information.
