1. This form is designed to be customized to accommodate your business look and feel. WAVES for Success has created this form because we understand the unique needs of today’s teenage employees. On the retaining, motivating, educating, and recruiting page, we have pertaining default questions for each topic. Modify freely as it relates to your challenges and opportunities. 2. Remember, what gets measured will improve! a. Reiterate this to your crew. The overall purpose of this form is to show that you as an employer care for the success of your staff. 3. Allow a week for the completion of this evaluation form. a. Give your teen employee this form and have them self-grade it first. Once it’s self graded by the employee, have the supervisor grade it from their point of view. Once the form is complete, have a meeting to review with both the employee and supervisor.
