3.0 Hearings and Appointments
3.1 Notice of Intent DEP 190-512 Applicant: Linda Fossile Owner: Linda Fossile Location: Lots 24 & 25 Lakeshore Drive Project Description: To construct a single family house, with site improvements including driveway, utility services, grading and landscaping. The project will include improvements of the on site riverfront area. A buffer zone planting plan has been designed, which includes tree and shrub plantings along the brook. Wildlife Habitat study was filed with previous plans. The original Order had expired. Paul motioned to close the public hearing and issue an Order of Conditions. Motion seconded by Minot, with all in favor, 7-0
Page 1 of 2 3.2 Notice of Intent DEP 190-514 Applicant: Alan and Linda Simoneau Owner: Alan and Linda Simoneau Location: 122 Chapin Road Project Description: Abandonment of existing septic system to the municipal sewer system. Steve Poole presented updated plans reflecting the easement across 83 Brigham Street. The original plan was to cross the brook. Paul motioned to close the public hearing and issue an Order of Conditions once number is assigned by DEP. Motion was seconded by Minot with all in favor, 7-0.
3.3 Notice of Intent DEP 190- Applicant: Jacobs & Buteau, Inc. Owner: Jacobs & Buteau, Inc. Location: Marycate Lane Project Description: Replacement of collapsed culvert and portion of headwall. Tom DiPersio Jr. said the road has a large sinkhole and the headwall is collapsing. Pipes have rotted out. Tom presented new plans with updated erosion controls. They will work around water mains. Minot motioned to continue to December 6, 2007 with a revision showing bags for the dam and a Wildlife Habitat Study. Motion was seconded by Paul with all in favor, 7-0.
3.4 RDA HUD 348 Applicant: Sherry Keith Owner: Sherry Keith Location: 480 Main Street Project Description: Remove two large maple trees. Both trees have limbs overhanging the house. One of the maple trees is dead and infested with carpenter ants. They will also remove limbs from a very tall pine tree which is located approximately 20’ from the house. In the driveway, limbs overhanging utility wires will be removed. Minot motioned to close the public hearing and issue a Negative Determination, seconded by Paul. All in favor, 7-0.
4.0 Commentary DPW’s General Order of Conditions expired in 2005. Request that they file a new Notice of Intent. They must come into compliance on the Cox Street property.
