 Completed forms are to be handed to your Coach/Manager before leaving the Cap Presentation. Please ensure that you have actually tried on the clothes before ticking the correct size boxes below.  Players unable to attend the Cap Presentation function must deliver this form to the Secretary at 6 Muir Court, Warner by 1st November 2016, where clothing sizes can be determined and cap collected. Please ring 0431 100 457 to arrange your visit.  PLAYERS are issued TWO playing shirts, TWO playing pants, ONE travel shirt, ONE pair of travel shorts, ONE travel cap, ONE wide brimmed hat and ONE backpack. A long sleeved playing shirt is available as an option (there is a limited number of these).  OFFICIALS are issued TWO travel shirts, TWO pair of travel shorts, ONE travel cap, ONE wide brimmed hat and ONE backpack. If an Official last year, you only receive one travel shirt and one pair of travel shorts and no other items.  If you require extra clothing, write the EXTRA number required to the right of the item name.  If you are choosing to use clothing from the previous year, put a line through the category. * Supporters can order travel shirts & travel caps only.
