STUDENT OPINION OF INSTRUCTION SURVEY Fall 2011
Instructions for Instructors – Paper Survey
The following instructions are for classes doing the paper survey.
1. You will receive a card in your mailbox for each class doing a paper survey:
The card will have a label with the class information on it.
2. Identify a student as the student leader. Give the student leader the card, have them go to the division office and retrieve the packet. The instructor should remain in the class until the student returns with the packet. The instructor should complete steps 2 through 10 before leaving the classroom.
3. When the student returns, verify that the correct packet for that class has been retrieved. Also, check the course number on the first and last forms in the packet to make sure that the contents are correct. If not correct, the student should return the packet to the division office and retrieve the correct packet for that class.
4. Do not have students fill out the surveys if the course or instructor is incorrect on the forms.
Revised: 07/26/11 5. The course identification and instructor’s name are coded into the bar code box at the bottom of the form. Remind students not to mark over this area.
6. The survey should be completed with a Blue or Black Ink if possible. The scanner may not read pencil or decorative/colored ink. All responses must be marked within the box provided for that question.
7. Explain to students how to mark the survey. Impress upon students the importance of filling out the survey neatly and accurately.
Mark an “X” inside the box next to your answer choice. If you would like to change your answer, completely fill-in the box containing the answer you want to “erase”. Then, mark an “X” in the box of the new answer choice.
(The Class Climate scanner identifies the answer choice as the box with a lesser amount of ink inside as in the above example.)
Revised: 07/26/11 8. Comments must be recorded inside the box provided. Comments written outside the box will not scan. Comments are distributed to faculty as they are written. If you do not wish your handwriting to be recognized, you may want to print.
9. Remind students NOT to put their name on the survey and emphasize that the survey is anonymous. Survey results are processed by the Office of Institutional Research and distributed to faculty after final grades are submitted.
10. Students should answer Section 2 only if this is a lab class.
11. Once you have verified that you have the correct survey packet and have given the instructions, YOU MUST LEAVE THE CLASSROOM.
12. The results will be distributed by email about mid-January. The email will be from Delgado Community College. The name of the course will be in the email subject line. If you do not receive your results by February 10, please send an email to: [email protected].
If you are missing any reports, you must supply your division, course §ion to Institutional Research in order for us to locate them. It is important that you notify us right away. Paper survey forms are kept only for a short period of time and are then destroyed correctly.
