You Will Receive Important Information Required by the Truth in Savings Act Electronically

This disclosure relates to your enrollment in Farmers State Bank’s “estatements” (electronic statement delivery).

*Please read this notice carefully and keep a copy for your records*

By enrolling, you agree to receive your statements electronically (via email).

Upon enrollment, you will no longer receive statements in the mail for the account(s) you have selected.

What are estatements?

By selecting “I Agree” you choose to receive your periodic statement(s) on all accounts that you presently or may in the future have enrolled electronically rather than on paper. You also authorize Farmers State Bank to send you any documents or notices that we are required to provide to you under any law or regulation electronically. This agreement is made between you and Farmers State Bank and acknowledges your request and consent to receive statements and notices for your accounts by electronic delivery. These electronic statements and notices are called “estatements”. We reserve the right to revoke your consent to receive disclosures/statements/notices electronically at any time. No fees will apply.

By enrolling for estatements, you are choosing to receive your statements and notices by email. Once enrolled, you will not receive any further statements or notices by U.S. Mail. Your bank statements and notices will be available for 18 months. At any time, the information will be available upon request by contacting us at 260-463-7111 or 888-492-7111 and a fee of $5 per statement may apply. When your statement is available, you will receive an email to that effect at the email address we have on file at that time. To ensure your ongoing receipt of estatements, you agree to notify us immediately of changes in your email address. In the event the email cannot be delivered to the email address on file, we will attempt to contact you via telephone or U.S. Mail. You will have 18 months from the time of the email within which to view, print, or archive your statement. The statement will also be available for 18 months by signing on to our internet banking service and choosing estatements. Your images will be included with your statement unless you have a free checking account. You may also unenroll at which time you will resume receiving your statements or notices by U.S. Mail. No additional fees apply to resume receiving your statements or notices by U.S. Mail. Note: If you do not sign on to your internet banking account for 366 consecutive days the system will delete your internet banking ID for security purposes. Upon the deletion of your internet banking ID, you will begin receiving your statements or notices by U.S. Mail with no additional fee. If you should need a prior statement one will be made available to you and a fee of $5 may be assessed.

What are the technical requirements?

To receive this information electronically, you will need access to a computer that satisfies the minimum requirements for electronic statements/notices which include:

. A computer with the ability to receive email with attachments

. Adobe Acrobat Reader 10.0 or higher (can be downloaded for free at http://www.adobe.com/ products/acrobat/readstep2.html)

. Internet Explorer Version 9.0 or higher Firefox Version 28 or higher Google Chrome Version 35 or higher Safari Version 7.0 or higher for Mac machines Safari Version 5.1.7 or higher for Windows machines

. 128 bit encryption

. Internet access . Valid email address

During the enrollment process, you should click the link to see a sample document to ensure your computer meets all the technical requirements. Additionally, you will need access to a printer capable of printing graphics (images) or the ability to download the information, in order to keep copies for your records. By selecting, “I Agree” you will accept the electronic fund transfer service with electronic disclosures.

By selecting “I Agree”, you accept responsibility for obtaining, installing, maintaining and operating all computer hardware and software necessary for accessing and successfully performing all online internet services. Farmers state Bank has no liability or responsibility for any errors or failures relating to the malfunction or failure of your hardware or software. You also agree to keep your password private; do not disclose your password to others and do not write your password on anything that can be viewed by others. You will be required to change your password on a regular basis. Farmers State Bank has no responsibility for establishing the identity of any person who uses your password.

Is there a fee for estatements?

Currently there are no service charge fees for the use of estatements. However, you agree that Farmers State Bank has the right to institute or change the fees for estatements upon thirty (30) days prior written notice to you. In addition, Farmers State Bank has the right to amend these terms and conditions from time to time providing you with written notice of the amended terms and conditions.

Upon enrolling for estatements, Farmers State Bank will waive the monthly fee for our bill pay service. We will need a completed bill pay application on file which can be obtained from any FSB office or on the website under the Online Banking tab. If you do not use the bill pay service for 3 consecutive months, bill pay may be removed from your online banking. If you choose to unenroll in estatements, the monthly bill pay fee will be reapplied.

How do I enroll for estatements?

To enroll for estatements you will need to sign on to our internet banking and click the estatement tab. Under step #1 you will choose which accounts you wish to enroll by clicking details. The default is all accounts. If you do not want all accounts remove the check marks next to the accounts you do not want to receive electronically and click save settings. Under step #2 you will verify your email address. Under step #3 you will enter a security phrase that will appear in the subject line of emails notifying you that your Estatements are available. Under step #4 follow the directions to enter the passcode. Under step #5 you will read the disclosure, click the link to see a sample document, and then check the box next to “I agree to the listed terms”. To complete the enrollment process you will click Enroll Now.

How do I make changes or add accounts?

To change which accounts are enrolled or enroll new accounts, you will need to sign on to our internet banking and click the estatements tab. Under the Documents and Settings tab you can add or remove check marks next to accounts and then click Save Settings. Accounts with check marks next to them you will receive electronically and accounts without check marks next to them you will receive by U.S. Mail. To review or print the disclosure, click the Disclosure tab. To change your email address or your security phrase, click the Email tab.

How do I unenroll?

To unenroll, you will need to sign on to our internet banking and click the estatements tab. Under the Documents and Settings tab you will need to remove the check marks next to the accounts and then click Save Settings.
