No.: BI-T-602-ARC Addendum 1 Date: Monday, Sept. 12, 2011 BI-T-603-ARC Web Advertisement Date: Wednesday, Aug. 31, 2011 Selection Type: Major Capitol Project Consultant Selection General Statement: In accordance with the requirements of CGS §4b-55 through §4b-59, the Bureau of Design & Construction Department of Construction Services, State of Connecticut, advertises for Requests for Qualifications for the Major Capital Project Consultant Services as specified below. Consultant Services: Architect and Consultant Design Team Contract Number: BI-T-602-ARC BI-T-603-ARC Contract/Project Title: Hammonasset West Beach Improvements Project Location(s): Hammonasset Beach State Park 1288 Boston Post Road Madison, CT 06443 Cost of the Work: $ 5,830,000 User Agency Name: Department of Energy & Environmental Protection (DEEP) Project Delivery Design-Bid-Build (D-B-B): Architect/Engineer consultants shall be selected to develop Method: design and construction documents for the State to publically bid the Major Capital Project. The general contractor that is the "lowest responsible and qualified bidder" is then awarded a contract by the State to build the Project. Construction Manager At Risk (CMR) - Guaranteed Maximum Price (GMP): Architect/Engineer consultants shall be selected to develop design and construction documents in the cooperation with a CMR for a Major Capital Project. The CMR shall publically bid the project elements and enter into a GMP contract with the State. After consultation with and approval by the commissioner, the CMR shall award subcontracts to responsible qualified subcontractors submitting the lowest bids to build the Project. Project Description: The Department of Construction Services, for the State of Connecticut Department of Energy & Environmental Protection, is seeking an Architect/Engineer (A/E) Consultant Team to Design a New Multi-purpose Building. The new Building will be replacing the existing toilet building and will also be including the following:
Toilet and shower area Lifeguard Station and Communication Center Concession stand
Associated with relocation of the new facilities will be parking lot renovations, an expanded shaded picnic area, boardwalk connection from the new facilities to the ADDENDUM NO. 1 CT DCS PROJECT NO. BI-T-603-Arc Page 2 of 5 RFQ ADDENDUM NO. 1 DATE OF ADDENDUM: September 12, 2011
existing beach boardwalk, sand dune creation between the new facilities and the Sound as well as the demolition of the existing toilet and west beach area concession buildings. Project Description The DEEP views this as a site circulation (vehicular, bicycle, pedestrian) project (continued) : within which, is a building component. Circulation boundaries are the camp store to the northwest and the mid boardwalk crossing to the southeast.
Designated Services: The Architectural firm must have extensive and comprehensive experience with plumbing, mechanical, civil site, landscaping, borings/geotechnical engineering, innovative green building and energy efficiency technologies are considered high priorities for this project along with some environmental beach sand dune restoration.
Affidavits Submittals: The Affidavit Requirements for Formal Contracts shall be determined by information found under the Ethics Affidavits & Legal Forms link at the top left of the CT DPW home page: www.ct.gov/dpw . (Click on “introduction” see chart. Also click on “General Advice…”).
The Submitter shall provide, in the Qualifications Submission, the “Ethics Affidavit”, if the contract fee is expected to exceed $500,000. The Consulting Affidavit and Disclosure Affidavit should be provided within 21 days after the notice of selection. At the time of the Contract execution, the Consultant shall be required to sign the “Gift and Campaign Contribution Certification” if contract fee is equal to or greater than $50,000. The Gift and Campaign Contribution Certification states that you, your company, and specified other individuals have given no gifts to DCS personnel and other individuals set forth in the Certification. For the purpose of signing the Certification “the date DCS began planning the subject project or services” is the date noted below. Pursuant to Connecticut General Statute 4-252(d) any bidder, proposer or person who responded to a request for qualifications for a contract with a value equal to or greater than $50,000 who does not make this certification at the time of the contract execution shall be disqualified. The noted $50,000 or greater value is based on Executive order #7C dated July 13, 2006. No forms, other than those posted on the DPW web page, shall be used by a consultant or design/builder.
With regard to a State contract as defined in P.A. 07-1 having a value in a calendar year of $50,000 or more or a combination or series of such agreements or contracts having a value of $100,000 or more, the authorized signatory to this submission in response to the State’s solicitation expressly acknowledges receipt of the State Elections Enforcement Commission’s notice advising prospective state contractors of state campaign contribution and solicitation prohibitions, and will inform its principals of the contents of the notice. (See, SEEC Form 11 is available on the State Elections Enforcement Commission Website at www.ct.gov/seec and clicking in their ‘Forms” link).

Qualifications This Qualification Based Selection (QBS) process is based on comparing and evaluating Based Selection weighted criteria of qualifications and performance of Firms in relation to work of similar scope (QBS): and complexity that is required for this specific contract. The Contract award is based first on a selection of the highest qualifications and second on the negotiation of a fair and reasonable fee. For the screening phase of the this selection the following are the rating point weights for the screening criteria categories:
