The challenge starts February 3, 2009 through April 28, 2009. If you would like to be a part of the “Biggest Loser District Team Challenge” you will need to do the following:
1. Each team needs to have a person designated as a Captain and one designated as a Co-Captain. 2. The $20 entry fee will be collected by your designated team captain as soon as your team is established. NO ONE IS TO SEND THE ENTRY FEE IN THE MAIL! Captains are to collect all $200 from their team and email to arrange pick- up or delivery with the District Nurse covering your site. Once you pay your fee, you are committed and there are no refunds. 3. The closing date to be a part of the “Biggest Loser District Challenge” is Wednesday, January 28, 2009. You must be on an assembled team by this date. Fees are due no later than Friday, January 30, 2009 by noon. We encourage you to contact the district nurse covering your site as soon as the $200.00 is collected and not wait until the deadline. 4. Your initial weigh in must be done on Tuesday, February 3, 2009 and be witnessed by your team captain. Captain’s weight must be observed by the Co- Captain. Captains must keep all weights confidential. 5. Team captains will weigh in their team members EVERY TUESDAY STARTING ON FEBRUARY 10, 2009. This should be the first weigh-in that shows a weight loss. CAPTAINS WILL REPORT THE WEIGHTS OF THEIR TEAMS TO KRISTIN STEWART RN VIA EMAIL AT [email protected] on 2/17, 3/3, 3/17, 3/31, 4/14 and 4/28. Do not report weights to Kristin every week, only on above dates. Even though your captains will not be required to email the results of your weight loss every week, you are still required to weigh in every Tuesday. It is important to keep you on track and accountable! 6. Each participant will receive the percentage of weight loss ranking via email on the Wednesday after the dates listed in number 5 above. 7. To keep the suspense on who our Biggest Losers are, we will not send out a team ranking on 4/29. This information will be announced at our celebration on 5/6/09. 8. Review the rules on the other attachment and make the commitment to improve your health status.
