Acute Services

Emergency and elective hospital services are provided by our acute services, which includes inpatient, outpatient and day case care. There are two district general hospitals within Ayrshire: University Hospital Ayr and University Hospital Crosshouse. These hospitals provide a wide range of acute services:

 University Hospital Ayr

University Hospital Ayr provides medical and surgical services on an inpatient, day case and outpatient basis. It is the main Accident and Emergency service for South Ayrshire. It provides a number of Ayrshire-wide services including Vascular Surgery, Ophthalmology and Audiology.

 University Hospital Crosshouse

University Hospital Crosshouse provides medical and surgical services on an inpatient, day case and outpatient basis. It is the main Accident and Emergency service for East and North Ayrshire. Inpatient paediatrics, the Ayrshire Maternity Unit and the main Laboratories for Ayrshire are on the Crosshouse site.

Community, mental health and learning disabilities services

Following the introduction of Health and Social Care Integration, the operational delivery for the range of community healthcare services and mental health and learning disabilities services is through the newly created Health and Social Care Partnerships and the Integrated Joint Boards:

Community Services

General medical and dental services are provided throughout Ayrshire and Arran by general practitioners, dentists, community pharmacies and optometry practices. Community nurses, health visitors and Allied Health Professionals are all involved in providing care within our local communities.

Out-of-hours general medical services are provided by Ayrshire Doctors On Call (ADOC) within the community. Mental health and learning disability services

Inpatient mental health services, including the Intensive Psychiatric Care Unit, are largely delivered at Ailsa Hospital in Ayr with further mental health inpatient services provided at University Hospital Crosshouse

Learning disability services are provided from Arrol Park in Ayr. Elderly mental health inpatient services are provided from Ayrshire Central Hospital in Irvine, and East Ayrshire Community Hospital in Cumnock. Community based services are provided throughout Ayrshire for the range of clinical groups: adults, child and adolescent, elderly and addiction services.

Building for better care

We are investing £27.5 million to provide fit for purpose front door services for University Hospitals Ayr and Crosshouse, in order to improve emergency and urgent care for patients who come to hospital for anything other than a scheduled appointment.

North Ayrshire Community Hospital – Woodland View

We are investing £47 million to provide a 206 en-suite bedroom integrated mental health and community facility, including older people’s long term care and rehabilitation. This development will bring together a full range of outpatient and inpatient facilities. Section five: Post Details

2. Job Purpose To utilise expert knowledge base and clinical competence, to support multi-professional teams within NHS Ayrshire & Arran, ensuring a high standard of clinical care, in accordance with the Nursing and Midwifery Council and Health and Safety legislation.

To work across role and organisational boundaries, as an integral member of the Acute Care Advanced Nurse Practice Team throughout NHS Ayrshire & Arran. Supported by protocols to perform clinical assessment, initiate investigations, determine differential diagnosis and undertake appropriate first line interventions;

To provide leadership, clinical support and advice to nursing and junior medical staff, making appropriate referrals to, and co-ordinating the contribution of other members of the multi-professional team;

To facilitate and participate in audit and quality assurance programmes, promoting evidence based practice within NHS Ayrshire & Arran.

To provide expert clinical care and management of both the stable and acutely unwell adult patient and his/her family, making complex, autonomous decisions using advanced skills and knowledge, having overall responsibility for the assessment of care needs and the delivery of advanced nursing care, to own caseload at a level equivalent to that of junior grade medical staff.

To provide specialist education, training to nursing and junior medical staff within NHS Ayrshire & Arran and to act as a role model, encouraging high standards of patient care. 3. Dimensions The Acute Care Advanced Nurse Practitioner carries out an expanded role which includes physical assessment, diagnosis of conditions, nurse independent prescribing and planning of care or onward referral to other members of the multi-professional team as appropriate, during normal and out of hours working.

The post holder works as an autonomous member of the multi-professional team within NHS Ayrshire & Arran and is the first responder to patients requiring clinical assessment and intervention. If required, supervision and support is provided through senior medical colleagues on the team.

The post-holder will also, working under the direction of a Medical Consultant and in partnership with other medical junior grade staff, share the clinical responsibility for a service.

The Nurse Consultant (Acutely Unwell Adult/Advanced Practice) is responsible for professional development and achievement of clinical competence for this role.

The Acute Care Advanced Nurse Practitioner has responsibility for the assessment of care needs and the delivery of optimal care to own caseload.

The post holder will have specialist knowledge of the needs of patients across a range of clinical issues, underpinned by the theory and practice acquired through an in-house education programme.

The post holder acts as a resource to other nursing and medical staff.

The post holder is not responsible for overall management of the budget but is responsible for ordering supplies for own use and provision of advice regarding specialist nursing equipment. 4. ORGANISATIONAL POSITION

Role of Department The overall role of the department is to work across role and organisational boundaries, to provide competent nursing input to work as an integral member of the Acute Care Advanced Nurse Practice Team throughout NHS Ayrshire & Arran. This is multi-faceted role, which encompasses care of both the stable and acutely unwell adult in-patients.

The Acute Care Advanced Nurse Practice Team incorporates a generic multi-professional team approach to clinical care throughout NHS Ayrshire and Arran. The Emergency Response Team (ERT) is an in-patient rescue service that liaises with the Acute Care Advanced Nurse Practice Team to actively identify and respond to patients who are identified as being at risk of acute deterioration. It facilitates an improved system of communication and prioritisation, to provide safe and effective patient care based on clinical need.

The ERT and Acute Care Advanced Nurse Practice Team sits within Acute Services, promoting patient centred services, based upon local and National strategies, taking account of NHS Ayrshire and Arran’s clinical, corporate and staff governance agenda.

Working collaboratively the department assesses, plans, implements and evaluates individualised programmes of care. The department works to promote a culture of openness and honesty and provides a learning and stimulating environment where staff can flourish.

6. Key Result Areas

Clinical  To work closely with medical and nursing colleagues, to ensure that patients receive appropriate assessment, investigation and treatment during and out-with working hours;  Provide an appropriate response to calls regarding patients from ward areas outside normal working hours;  To triage patients, prioritising clinical need, initiating treatment supported by protocols and refer those patients who require medical intervention;  To be responsible for the management of own caseload during the span of duty, and assist in medical management of Acutely Unwell patients. This will be in partnership with the multidisciplinary team and will include the patients’ journey from admission to discharge.  Utilise advanced knowledge and highly specialist clinical skills to assess, plan, implement and evaluate programmes of care in order to deliver the highest possible standard of care to patients and their families within the confines of available resources;  Prescribe and administer intravenous fluids and medications according to Patient Group Direction (until independent nurse prescribing course completed);  Assess need for medication and undertake nurse prescribing duties in line with organisational policy;  Be involved in a range of clinical responsibilities, which will improve and maintain the experience of patients within NHS Ayrshire & Arran  Routinely undertake clinical assessment of acutely unwell adults and stable patients including history taking, clinical examination and arranging relevant investigations;  Routine management of stable patients including regular monitoring, communication with ward staff and medical staff where clinical concerns arise and responsibility for the discharge process;  To carry out venepuncture, cannulation, arterial blood gas sampling and analysis, IV preparation and administration, management of fluid balance, undertake12 lead ECG and basic interpretation;  Initiate treatments, supported by written protocols and guidelines eg. commencing intravenous infusions, anticoagulation, ordering emergency chest x-ray and cross-matching for blood transfusion;  Liaising with laboratory and radiology departments and ensure that results are acted upon promptly;  Lead and act as a specialist resource by promoting the service and increasing the awareness of the care and management of medical conditions in the acute hospital setting to enhance standards of care;  Act as a role model for staff, giving clinical guidance, advice and support  As an Acute Care Advanced Nurse Practitioner, give advice and support to medical colleagues, Ward Manager and team leaders on medical clinical matters and act as a resource to nursing staff.  As an Acute Care Advanced Nurse Practitioner, support the nursing team to ensure that the appropriate clinical skills and competencies are used effectively to meet the needs of both acutely unwell and stable patients and their families;  Demonstrate expert clinical knowledge and skill, by keeping up to date with evidence based; practice, through continuous professional development, to maintain high levels of clinical practice  Share responsibility for clinical and staff governance with medical colleagues, Ward Managers and nursing staff;  Report complaints, incidents and accidents via the appropriately identified channels and assist managers in their investigation and resolution. Professional  Practice at all times within the Nursing and Midwifery Council Code of Professional Conduct.  Develop the role by using evidence-based practice and continuously improve own knowledge, following PREP guidelines.  Conduct clinical evidence based practice in accordance with clinical guidelines and standards.  Act as an exemplary professional role model for nursing services.

Leadership and Management Manage own time to meet the various demands of the role, including clinical, educational and administrative elements. Plan and organise education and training events for staff.

Participate in internal and external committees relating to services in order to contribute to the development of future medical service provision.

 In partnership with the multidisciplinary team, develop, implement and maintain policies, procedures, standards and protocols of Acute Care Advanced Nurse Practice Team; ensure adherence to these in order to deliver the highest possible level of patient care within available resources;  Ensure that patient needs are assessed, care planned, implemented and evaluated, and that there is consultation and involvement of patients/carers;  Ensure that all nursing staff and members of the multidisciplinary team are aware of, and work within, local, directorate and Division policies and procedures to ensure that safe working practices are maintained for both patients and staff;  Identifying, minimising and managing interpersonal conflict, maintaining the trust and support of other healthcare professionals and disciplines  Report all incidents as per incident reporting policy;  The role will integrate into the existing clinical structures to support a team of professionals. Research and Audit  Critically evaluate current practice, developing innovative ideas to improve effectiveness and efficiency of care;  Develop clinical audit programmes to support ongoing assessment of practice to ensure improvements in care and best practice;  Evaluate newly generated research findings and adapt them for practice using advanced knowledge and skills and disseminate findings internally to influence best practice;  Contribute to and take an active role in key clinical research projects;  Encourage and facilitate research by the multi-disciplinary team;  Participate in the systematic review of guidelines and protocols for practice.

Education  In support of the Line Manager, participate in the on-going personal development and professional education of all members of the Acute Care Advanced Nurse Practice Team nursing team utilising mentorship, objective setting and appraisal, enabling education needs of nursing staff to be met;  Participate in the development and delivery of specific educational programmes for the ongoing professional development of nurses;  Participate in the provision of study days and in-service education programmes for staff within NHS Ayrshire & Arran, including presenting in classroom settings;  Direct and support the provision of clinical practice for pre registration and post registration learners, to fulfil curriculum requirements and ensure that appropriate educational opportunities are provided.

8. Assignment and Review of Work  The post is largely clinically based requiring self-direction and discretion in response to the clinical needs of the patients and their families as well as the educational needs of the staff;  Work is generated by service developments, advances in practice and research;  Review will be annually by formal performance appraisal, personal development plan and objective setting.  The Post Holder will have a Professional Personal Development Plan and be reviewed by the Advanced Nurse Practitioner – Clinical Manager.  Initiate and follow through appropriate procedures when a breach of policy occurs.  Work will be generated and allocated to the post holder following the multi-professional handover, and at designated times during out-of-hours;  During working hours clinical work will be generated and assigned by the multi-professional teams.  Aspects of work related to service development and training will be assigned through senior management teams 9. Decisions and Judgements As an autonomous professional, the post holder is expected to:

 Make complex clinical and professional autonomous decisions on a daily basis regarding the nursing and medical management of patients. This will also include the provision of advice to the multidisciplinary team;  Justify clinical decision making with regard to patient’s health care, through stringent monitoring of the patient’s condition and acting on clinical judgement;

 Act as patient’s advocate to ensure their rights are upheld;  Freedom to act and make clinical decisions is guided by precedent and clearly defined divisional policies, protocols/procedures and codes of conduct in accordance with NMC regulations, Clinical and Staff Governance Framework;  Post holder has the autonomy to proactively identify and resolve operational and clinical issues that impinge on service needs/ patients safety;  Assessment of patients complex needs (physical, psychological, social and spiritual) to establish change in condition, inform clinical decision making and plan care;  The ability to quickly assess and respond to patient needs in emergency situations.

10. Communications and Relationships The main purpose of communication would be regarding issues relating to patient care. There will be a frequent requirement to receive and communicate complex, condition related information tactfully and in a manner which can be readily understood by patients/carers and to handle sensitive information in a manner not liable to offend or antagonise.

The postholder will:

 Use appropriate communication skills including motivation, negotiation and persuasion to achieve optimum outcomes for service users;  Demonstrate empathy, sensitivity, support and reassurance when communicating with patients, relatives and staff, particularly in relation to breaking bad news and dealing with emotionally challenging situations, dealing with situations where patients do not easily understand their care packages due to cultural, language, physical or learning disability or where there is denial or anger;  Communicate effectively to help establish good working relationships with colleagues, patients and their families, and the general public;  Liaise with other Health Care Professionals and disciplines from within and out with the organisation to support quality patient care;  Coach others in the use of effective communication and the interpersonal skills required to develop/improve performance;  The post holder would be expected to communicate with the department manager regarding nursing team issues and to liaise with the multidisciplinary team on service needs and requirements

12. Physical, Mental, Emotional and Environmental Demands of the Job

The post holder will be involved in clinical, research and education activities and may frequently be required to move between tasks at short notice to meet the needs of the service. There is a requirement for flexibility of working patterns and for the ability to deal with situations as they arise. Workload is unpredictable, with priorities changing constantly.

Physical  Frequently working in highly unpleasant working conditions such as having contact with un-contained body fluids, foul linen,  Potential exposure to hazards such as verbal and physical aggression;  Prolonged light to moderate physical effort, including bending, walking, lifting, pushing and operating equipment is required on most shifts;  The post holder will be required to work within limited spaces, and will be required to assist immobile patients;  The post holder will be required to be able to initiate appropriate emergency care.  The post holder needs to adapt to the shift pattern required and may be required to work a variety of shifts.

Mental  Concentration required due to the nature of the Acute Care Advanced Nurse Practitioner role, where there is a requirement for high levels of concentration and for absolute accuracy for clinical tasks;  Clinical decision making which may affect a patients care;  Frequent direct and indirect interruptions from patients, relatives and the multidisciplinary team;  Prescribing and administering treatment under nurse prescribing;  Balancing the various aspects of the post;  Time management;  The post holder will be required to use own initiative and be able to make decisions with minimal supervision.

Emotional Effort/Skills  Discussing complex clinical issues with the multidisciplinary team and patients;  Impart bad news to patients and families;  Caring for and supporting patients and their families following receipt of bad news;  Caring for the terminally ill patients;  Caring for Acutely Unwell and stable patients with complex problems;  Dealing with parents/carers/relatives with severely challenging behaviour;  There is a requirement to deal with distressed/anxious/worried/bereaved patients / carers/ staff in a professional and sensitive manner on a daily basis;  There is a requirement to deal with complicated family dynamics and high levels of public expectations;  There is a requirement to on occasion work in hostile and emotive atmospheres, which may result in the need for sudden intense effort and concentration;  The post holder will be required to deal with complaints, involving meeting the complainant and seeking to address their concerns, whilst supporting staff involved.
