TC-25h THIRD REMINDER NOTICE OF RECERTIFICATION (Termination of Section 8 Assistance)

Use this form when a resident household does not respond to their “First Reminder Notice of Recertification” or their “Second Reminder Notice of Recertification” (TC-24h). The TC-25h form should be sent on the 1st day of the month that is 60 days prior to the Annual Recertification date. This is because “First Reminder Notice” is sent 120 days in advance of the annual recertification date. The resident must respond by the 10th of the month that is 60- days prior to the Annual Recertification date in order to avoid paying market rate rent. This will allow for more time to complete the required certification paperwork. Please see the instructions below for completing the form. Be sure to record the date the TC-25h was sent for HUD compliance reasons.

If the resident fails to comply with this notice (TC-25h), their rent will be will be raised to market rate rent effective on their Annual Recertification date and their Section 8 assistance will be terminated. The resident will remain at market rent until he/she complies with the annual recertification request. In properties with a limited number of Section 8 slots, the slot should be given to another qualified resident or applicant effective on the Annual Recertification date stated. (Please see the Manager’s Handbook Policy under “Leasing and Occupancy” for further instructions.)

These instructions should be followed:

1. Insure that the first part of the form is completely filled out with all pertinent information.

2. Record the applicable dates (see table below).

3. Fill in the name of the “Head of House”.

4. Fill in the month that is 60-days prior to the annual recertification date. For example: 9/1/03 annual recert date; July 10, 2003, the final reminder date.

5. List the full market rate rent and effective date. The effective date of the higher rent is the annual recert due date. In the above example, this would be 9/1/03.

6. Be sure to enter the telephone number of the site Management Office.

7. Sign the letter, make two photocopies of the original and deliver to resident household per instructions on the form. Remember that the original letter (notice) must be mailed and a photocopy must be hand delivered. Please be sure to annotate how the form is delivered to the resident and by whom, both on the original and the photocopies.

8. Place one of the photocopies in the resident file with the certification paperwork NOT under the “Notices” section. When a file is audited for compliance, certification paperwork is checked to see if the resident household received the proper (required) HUD notices.
